Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00052900 Site Location: 16777 HOWLAND RD <br />Activity Date <br />Activity Summary <br />Recorded by <br />11/02/2020 3:05 PM Responded to page from Laura Sauers (EHD) for a 50 gallon release of MURO <br />Phosphoric Defluorinated Acid at 16777 Howland Rd in Lathrop. Complainant is: <br />Ryan Mock and his phone number is 209-328-0083. At 3:13 PM Received Cal <br />OES Control #: 20-6105. An above ground storage tank is leaking an estimated <br />50-gallon release of Acid - Phosphoric Defluorinated Type from underneath the <br />tank onto gravel and soil. Responsible Party is handling the containment and <br />cleanup. Leak has not stopped. <br />Called Ryan Mock (Environmental Manager at JR Simplot) 209-328-0083. Ryan <br />Mock reported the following: <br />No waterways were impacted <br />No injuries <br />No evacuations <br />Leak is not stopped; is a slow leak <br />Does not know how much has released; estimates 50 gallons <br />- Tank Details: 49,680-gallon capacity; dimensions 30 feet wide by 30 feet tall; <br />Tank ID: 4927-TK449; Tank exterior: steel; Tank interior: rubber-lined; <br />- If tank were to fail, product would remain within the facility; tank is on a gravel <br />pad with concrete ring <br />- NRC and Fire Department were notified <br />- Facility is planning to offload tank product, clean the tank, inspect the tank, <br />then assess ground cleanup and remediate <br />I let Ryan Mock know that our after-hours inspector, Elianna Florido (EHD), will be <br />going to the facility. <br />3:32 PM Received Update from Cal OES: NRC report received: Reporting Party is <br />exploring options to remove the remaining material in the storage tank. <br />Searched Envision Connect and found this was for Simplot, which is under our <br />inventory. <br />Received a complaint form 5104: C00052900 from EHD front desk staff, Laura <br />Sauers (EHD). Added property owner information (found on parcel viewer) and <br />responsible party information to complaint. <br />11/02/2020 Discussed complaint with Lead Senior REHS Elena Manzo (EHD). MURO <br />11/03/2020 Updated Melissa Nissim, Elena Manzo, and Claudia Muro on the complaint. FLORIDO <br />Completed my 11/2/20 narrative. <br />11/03/2020 Date stamped and downloaded inspection photos. Generated photo log. Emailed <br />photo log and phosphoric acid safety data sheet to Claudia Muro, Elena Manzo, <br />and Melissa Nissim. <br />FLORIDO <br />11/03/2020 Discussed complaint with Elena Manzo (EHD). MURO <br />Amended Notice to Abate to include the tank capacity, ask to stop the leak, and <br />repair the tank.
