Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COBZW2HAW Site Location: 410 HAROLD ST <br />Activity Summary <br />Activity Date <br />Recorded by <br />10/26/2020 11:44 AM Called Officer Johnston, 916-505-6213. Left message. MURO <br />1:50 PM Called Lodi Police Department, 209-333-6728. Left message with <br />dispatcher with my contact info. <br />3:34 PM Received call from Officer Johnston who reported the following <br />information: <br />A boy found the lysol squirt bottle with the unknown chemical on an empty dirt lot. <br />He squirted himself and ran home to wash off the chemical. Later, another boy <br />went to grab the lysol bottle and accidentally squirted a girl in the face. The girl <br />developed blisters on her face and was rushed to the hospital in an ambulance. <br />The boy who was initially exposed to the unknown chemical was okay and didn't <br />develop blisters. <br />Officer Johnston also reported that Public Works told him that they would pick up <br />the chemical on Monday and would be taking it to Household Hazardous Waste <br />Facility. <br />Sent email to Maggie Park, (SJC Public Health Services) at mpark@sjcphs.org to <br />notify of this exposure. <br />10/27/2020 Completed narrative for 10-24-2020. MURO <br />Emailed Officer Johnston at gjohnston@lodi.gov to ask if the chemical waste was <br />picked up. I also asked if he could specify whether he meant Lodi Public Works or <br />SJC Public Works. <br />10/27/2020 Consulted Lead Senior Elena Manzo whether I should generate a complaint for <br />this incident. I was advised to generate one. <br />MURO <br />Generated complaint. <br />11/02/2020 Called Lodi Police Department, 209-333-6728. I spoke to Maria, who said that MURO <br />Officer Gaige Johnston was off duty at the moment. Maria provided the following <br />information: <br />Case Number: 206055 <br />Ambulance Phone Number: 209-236-8341 <br />Call came in at: 16:35 (4:35 PM) <br />Disposition of the unknown chemical: Booked in "Property" at Lodi Police <br />Department. <br />Maria transferred me to the "Property" unit. I spoke to Kim. Kim reported the <br />following: <br />- The unknown chemical is still at the Lodi Police Department (LPD), with no public <br />access <br />- The unknown chemical does not have a hazardous waste label, instead it has a <br />"biohazard" label <br />- the unknown chemical will remain at the LPD for about a year, until she <br />accumulates other wastes <br />11/02/2020 Meeting with Melissa Nissim (EHD), Elena Manzo (EHD), and Stacy Rivera (EHD) <br />regarding this complaint. <br />MURO <br />5105.rpt <br /> Page 2 of 3
