Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COJISLPEO Site Location: NEAR 11 W ATHERTON ISLAND <br />Activity Summary <br />Activity Date Recorded by <br />11/04/2020 Sent email to Juan Neira (SJAFCA) requesting clarification for the two samples MURO <br />taken for analytical lab testing titled as 'background and 'cofferdam' on the chain <br />of custody. <br />Received a phone call from Juan Neira, he reported: <br />Background sample = A water sample was taken from the water outside the <br />cofferdam <br />Cofferdam sample = A water sample was taken from the water within the oily <br />boom. (In other words, oily boom water.) <br />Total oily boom waste generated = one 55-gallon drum. <br />They are waiting for EHD's disposition to dispose of this waste as non-hazardous <br />waste based on the lab results. <br />11/05/2020 Discussed the samples with Elena Manzo via emails. Later discussed in person MURO <br />with Elena Manzo and Lead Senior of Hazardous Wastes Garrett Backus (EHD). <br />The lab results for the samples of cofferdam oily boom drippings was accepted. <br />Facility can dispose of the waste generated (one 55-gallon drum) from the cleanup <br />as non-hazardous waste. <br />12/16/2020 Mapped location on envision connect GIS Map. MURO <br />12/16/2020 Discussed whether we need a disposal record with Elena Manzo before sending MURO <br />request. <br />Sent email to Juan Neira (SJAFCA); requested copy of disposal record for the <br />55-gallon drum of oily water and boom. <br />01/20/2021 Received email from Juan Neira (SJAFCA) inquiring what they need to provide for MURO <br />the boom waste disposal since the receipt will not specify the boom waste. Receipt <br />will contain "recyclable/non-recyclable material". Requested copy of disposal <br />record/receipt with statement stating boom waste was contained within the load. <br />09/15/2021 Emailed Juan Neira (SJAFCA) requesting disposal record and statement. <br />Juan Neira called me for clarification on what I was requesting. I explained what I <br />was requesting. Juan emailed me a copy of a disposal record and statement <br />stating the wastes generated from the incident were contained with the disposal <br />pickup. <br />MURO <br />09/20/2021 Printed remaining complaint items. Compiled complaint file. MURO <br />09/24/2021 Reviewed complaint to begin abatement procedure. Printed all related documents MURO <br />associated with this complaint.
