Complaint Investigation Summary Report #. 5105 COMPLAINT ID: C00050597 Site Location: 1550 S B ST #39 Activity Summary Activity Date Recorded by 10/04/2019 Met up on site with Mary Rosina. She she said that it was baby wipes that were AFONSKAIA clogging the line and they that had gotten it out and it was working properly now. He said that there is no tank there it is just an outlet. She said that the tank the the RVs are connecting to are up north before space 6. The line goes all the way down to what she called a cistern it looked like a lift station, it had a pump which she said went out to the pits. She said the pump had a shredder. I told her that the RVs cannot be connected to the septic system because of the chemicals and she said that it's the first time she's heard of it and she's been doing this for 20 years. She said that the RVs were using chemicals. I also told her that they can't have connections down hill of the tank because only liquid can go into the leach field at which point she showed the lift station. 10/04/2019 Called Marta to see if they were able to unclog the tank and get it pumped, she AFONSKAIA said they are still working on it. I asked her to send me a map of the place to be able to better interpret the permits on record. Spoke with Jeff about the complaint and he contacted Steve Downer at HCD and they said that they did not permit any holding tanks at that park. I emailed all my information to Steve Downer. He went to the park, he said that they cleaned the area up and that it was in fact a clean out and not a holding tank. And that he did not have any issues with the RVs being connected to the existing septic systems. I called Mary to see if I could go on site to verify it was fixed and she said she would meet me on site. There is no lift station in any of the permits on record. Spoke with Michael and he said that they can't have any connections after the septic tanks regardless of the lift station pump having a shredder and that it is considered illegal connections. I will verify with Mary on Monday and send a new notice to abate. 10/04/2019 Sent Daria Afonskaia (EHS) a copy of the notice to abate. GAITAN Discussed if I had given the responsible party a compliance date. I replied that I had verbally told the owner to fix the dump station immediately. 10/07/2019 Spoke with Marta about if she got my email requesting a copy of the map for the AFONSKAIA park. She said that the problem has been resolved at the clog was after space 29. I re-sent the email requesting the map and any pumping records that they have. She sent the email with the map. 10/10/2019 Called Mary, no answer left message requesting call back. Spoke with Michael AFONSKAIA and he said since we do not have records of the complete septic systems and the owner does not know as long as the surfacing sewage has been dealt with to abate the complaint. Abated complaint because surfacing sewage was cleaned up, the line was unclogged and the clean out was capped. 10/10/2019 Returned copy of complaint form to Dana Afonskaia (EHS). GAITAN 11/25/2019 Received note stating that I needed to provide billing address for mobile home GAITAN park. Found billing address and updated it on EC. Let Michelle Ozuna (Accounting) know that the information was updated. 01/23/2020 Received return to sender mail from AnnaMarie Taylor (Accounting). Looked for Dada Afonskaia (EHS) to discuss complaint. Talked to AnnaMarie about mailing address. Sent billing to the following address: 1310 MONUMENT BLVD.
