Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COCNCM5YD Site Location: 1007 S MAIN ST <br />Activity Summary <br />Activity Date Recorded by <br />02/10/2021 Routine hazardous waste inspection done 2-9-21. Hazardous waste generator BACKUS <br />operating permit created. Inspection report completed and mailed to facility. See <br />PR0546502 for the inspection report. <br />Mary Tran, CFO, created the CERS account and stated yes for hazardous waste <br />(HW) generator activity. She also filled out and posted the modified contingency <br />plan. Stericycle hauls their waste away but only one transport record was provided <br />for review. <br />Violations found: No HW determination for ImageMax Developer Cleaner. No <br />HW determination for dental materials such as teeth whitener, teeth impression <br />material, or adhesives. Missing recycling records for photo fixer waste silver <br />recycling, dental amalgam mercury universal waste and universal waste batteries <br />and e-devices. <br />Hazardous waste streams found: ImageMax Developer Cleaner, ImageMax <br />Developer, ImageMax Fixer, mercury-containing dental amalgam, universal waste <br />electronic devices, universal waste batteries. Empty nitrous compressed gas <br />cylinders are returned to the gas vendor. <br />Robert McClellon and Sandip Singh of the EHD were on-site at the same time, <br />conducting the Medical Waste Management Act (MWMA) inspection. <br />Wastes generated by the facility that are covered by the MWMA: Anesthetics such <br />as Articaine, Sedation pills such as Valium, and items contaminated with blood or <br />bodily fluids. <br />Mary Tran stated that dental tool sterilization is done using steam. Chemical <br />sterilization has not been done for seven years. <br />Mary Tran stated that they don't need to dispose of toothpaste or listerine. <br />02/10/2021 Reviewed CUPA inspection report. SINGH <br />I called Mary Tran for an update on the tracking records. She stated Harpal has <br />the records,I was provided with Harpal's phone number (925-383-4827).1 left a <br />message on Harpal's voicemail, there was no answer. <br />Received a call from Harpal. She stated she has the tracking records. We agreed <br />that she would send tracking records from April 2019 to present. The facility was <br />closed for months in 2020 due to thr pandemic. Harpal stated she will email the <br />records by the end of today. <br />Discussed inspection with Robert M. Drafted an OIR. Emailed Robert M. the OIR <br />for approval. <br />02/11/2021 Discussed the inspection report with Robert M. Revised inspection report. Emailed SINGH <br />the inspection report to Mary Tran at mary@mantecadentistry.com. <br />Updated complaint. <br />Received tracking records from Harpal via email. Reviewed records.
