Complaint Investigation Summary <br /> Report #. 5105 <br />COMPLAINT ID: COCNCM5YD Site Location: 1007 S MAIN ST <br />Activity Summary <br />04/14/2021 <br />Received a call from Harpal to reschedule the reinspection. She mentioned the <br />dental office is really busy every day besides on Monday's. I told her that I would <br />ask Robert M., then get back to her. <br />Rescheduled the inspection for Monday 3/15/21. A time has not been decided. <br />Emailed Harpal with the time of the reinspection. The reinspection is scheduled for <br />3/15/21 at 11 am. <br />Reinspection cancelled and rescheduled for 3/17/21 at 9:30 am. <br />Prepared for inspection. Reviewed complaint. Prepared OIR. <br />Manteca Dental Care - 1007 S Main St., Manteca - Complaint reinspection. The <br />reinspection was conducted with Program Coordinator, Robert M. We met with <br />Harpal Chandra, Accounts Payable, at the dental practice. The Stericycle manifest <br />for medical waste was available for review. Per Harpal, the last pick up was on <br />3/12/21 but she did not have a manifest available. She also stated Stericycle will <br />provide a biohazard sign for the cabinet door. A red biohazard bag was observed <br />in a container in the biohazard cabinet. <br />Reviewed complaint documents prior to inspection. <br />After the inspection I created a photolog. I emailed Harpal for March's manifest <br />and proof of the biohazard sign on the cabinet door. I informed her that the <br />complaint will be abated after EHD receives the corrections. <br />Received approval for the OIR from Robert M. Emailed OIR to facility owner. <br />Updated file. <br />Received OIR from CUPA. Reviewed OIR. <br />Complaint abated - field response - violations cited and corrected. <br />Activity Date Recorded by <br />03/08/2021 Called Harpal at 925-383-4827 to schedule reinspection. I went over the items that SINGH <br />we will be looking for. I reviewed the manifest from SINGH <br />Stericycle. Stericycle picked up the medical waste on 3/12/21. Harpal emailed a <br />picture of the medical waste container with a biohazard sticker, but the biohazard <br />sticker should have been placed on the cabinet door. I notified Harpal. <br />04/05/2021 Worked on the OIR. Emailed CUPA for an update. SINGH <br />04/06/2021 Received an update from CUPA. Reviewed CUPA OIR. Updated information on SINGH <br />OIR. <br />04/13/2021 I called Harpal to inform her that I have not received the correction for the <br /> SINGH <br />biohazard symbol on the cabinet.
