Decontaminate affected personnel after critical <br />first aid is given. <br />Obtain paramedic services or ambulance transport to local hospital. This procedure shall be <br />followed even if there is no visible injury. Calling 911 from a mobile phone will notify CHP <br />only; direct numbers to fire and ambulance are provided above. <br />Other personnel in the work area shall be evacuated to a safe distance until the SSO <br />determines that it is safe for work to resume. If there is any doubt regarding the condition of <br />the area, work shall not commence until all hazard control issues are resolved. <br />At the earliest time practicable, the SSO shall contact the Project Manager, or their designees, <br />giving details of the incident, and the steps taken to prevent its recurrence. The SSO shall <br />report any incidents to IncidentReport@gfnet.com <br />2. Person Overboard — Overwater Borinas: If a person falls overboard into the water while being <br />transported to the barge or back to shore, the boat should turn on the side of the person who fell <br />overboard and the captain should stop the boat engine (this will not apply if person falls overboard <br />while working on the anchored barge). Assign a look -out to indicate and track the position of the <br />person in the water. Release ring buoy for the overboard person to swim to. Perform an elliptical <br />or Williamson turn, if needed. Prepare for recovery of persons from the water and hoist a signal <br />flag. Have the overboard person place the ring buoy around their body under the armpits and tow <br />the person in slowly. Carefully lift the person into the boat or onto the barge by both arms with the <br />person's back facing the boat/barge. The person can also be pulled into the boat or on the barge <br />by the life vest with their back facing the boat/barge. Immediate first aid should be administered, <br />if required. <br />3. Fire or Explosion: Turn off all motorized equipment; evacuate working area; meet at designated <br />upwind location. <br />4. Earthquake: Turn off all motorized equipment; evacuate working area; meet at designated upwind <br />location. <br />5. Environmental Hazards: In the case hydrocarbon measurements indicate an explosive or potentially <br />explosive atmosphere as indicated by the 4 -gas monitor (greater than 10 percent lower explosive <br />limit), all equipment must be shut down and work will cease. Evacuate working area until readings <br />return to safe levels. Similar protocols will be followed for accumulation of carbon monoxide or <br />depletion of oxygen in the work environment. <br />6. Hazardous Material Spill or Release: Turn off all motorized equipment; evacuate work area in an <br />upwind direction of the spill or release; meet at designated upwind location. <br />7. Personal Protective Equipment Failure: If any site worker experiences a failure or alteration of <br />protective equipment that affects the protection capabilities, that person shall immediately leave <br />the Work Zone. Reentry shall not be permitted until the equipment has been repaired or replaced. <br />8. Other Equipment Failure: If any other equipment on-site fails to operate properly, the project team <br />leader and SSO shall be notified and then shall determine the effect of this failure on continuing <br />operations on-site. If the failure affects the safety of personnel or prevents completion of the work <br />plan tasks, all personnel shall leave the Exclusion Zone until the situation is evaluated and <br />appropriate actions taken. <br />berrrriYt t Flerrur rg <br />
