HASP — Project Vandelay — Stockton Upgrade Geotechnical Exploration Services <br />August 2019 <br />Project No. 066079 <br />Page 3 of 8 <br />Level of Protection: <br />✓ "Level D" including hard hats, safety glasses, hearing protection, high visibility safety vest, steel toe <br />boots, and nitrile gloves. <br />✓ US Coast Guard approved life vest when working adjacent to water or on the barge. <br />✓ Ring buoy with 90' of rope within 200' of work activities will be provided by Taber Drilling <br />✓ Lifesaving skiff will be provided by Taber Drilling. <br />✓ Air monitoring in the breathing space is not anticipated for this project. <br />✓ A 4 -gas monitor for periodic ambient air measurements near and downwind of the onshore borehole <br />locations using a photoionization detector (PID) and organic vapor analyzer (OVA) will be used. <br />Location(s) to be used: <br />When to use: All people must meet "Level of Protection" requirements while working at the site. <br />Overhead Power Lines and Underground Utilities <br />Care should be used when drilling adjacent to overhead power and/or communication lines. Vertical and <br />horizontal clearance from overhead power lines should be in accordance with OSHA guidelines, with the <br />minimum as noted below: <br />Nominal Voltage <br />(phase tophase) <br />Minimum Required <br />Clearance (feet) <br />600 to 345,000 <br />10 <br />345,000 to 750,000 <br />16 <br />750,000 to 1,000,000 <br />20 <br />The rig may be closer to communication (i.e., non -conducting) lines provided the location is acceptable to <br />the driller. The driller reserves the right to determine if a particular location is unsafe. In any case, disturbance <br />or damage to existing facilities is not acceptable. <br />Drilling should only be performed at locations marked and called into Underground Service Alert (USA) at <br />least 2 working days prior to the start of drilling. USA ticket numbers and expiration dates will be provided <br />on the first page of this plan. <br />In addition to performing underground utility clearance through USA, Gannett Fleming will retain an <br />underground utility surveyor to locate underground utilities in the area of the proposed landside borings <br />and SCPTs. Following a response from members notified by USA, the utility surveyor will visit the site to <br />mark underground utility locations on the ground with spray paint. <br />The SSO is responsible for checking that there are no utilities or special instructions/markings in the <br />proposed area of drilling. Minimum clearance from all marked utilities is three (3) feet. If markings are <br />noted, the SSO should relocate the borings to an acceptable location within the cleared area. If a clear <br />location cannot be located, the Project Manager should be notified and drilling of the location(s) postponed. <br />berrTnPtt Fleming <br />
