The following is an itemized list of hazardous materials violations that have not been
<br /> addressed for UNION POINT MARINA BAR & GRILL INC as of July24, 2017.
<br /> Open violations from June 22, 2017 inspection
<br /> Violation #3 - Failed to establish and implement a hazardous materials business plan.
<br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business
<br /> Plan shall be established and implemented for emergency response to a release or threatened release of a
<br /> hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for
<br /> materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and
<br /> pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa,ca.gov/,
<br /> enter the required information , and submit for review by the EHD.
<br /> Violation 94 - Failed to complete and/or submit the Business Activities or Owner/Operator Identification
<br /> page.
<br /> The Business Owner/Operator Identification form was not complete. The Secondary Emergency Contact
<br /> information has not been completed. This information must be current and submitted as part of the business plan.
<br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the
<br /> correct or updated information , and submit to the EHD for approval.
<br /> Violation #5 = Failed to complete and/or submit hazardous materials inventory for all reportable materials on
<br /> site.
<br /> A reportable amount of compressed carbon dioxide gas was found on site that has not been reported. Any material
<br /> that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into
<br /> the California Environmental Reporting System (CERS) at http://cers.calepa,ca, gov/, add the materials to the
<br /> hazardous material inventory, and submit to the EHD for approval.
<br /> Violation #6 - Failed to complete and/or submit a site map with all required content.
<br /> The site maps do not contain north indicators, drains, access and exit points emergency shutoffs, evacuation
<br /> staging areas, carbon dioxide storage and handling areas and emergency response equipment. Asite map shall
<br /> contain a north orientation , loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit
<br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and
<br /> emergency response equipment. Immediately log into the California Environmental Reporting System (CERS) at
<br /> http://cers.calepa.ca, gov/, upload the correct or updated information , and submit to the EHD for approval.
<br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened
<br /> release.
<br /> The emergency response procedures have not been submitted. A business plan shall include the following
<br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not
<br /> limited to, the following:
<br /> . immediate notification of local emergency personnel and the Environmental Health Department (EHD)
<br /> . procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to
<br /> persons, property, or the environment
<br /> . evacuation plans and procedures, including immediate notice, for the site
<br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the
<br /> correct or updated information , and submit to the EHD for approval.
<br /> Violation #8 = Failed to submit a training program or program is not reasonable or adequate for facility.
<br /> Atraining program has not been submitted. Training programs for all employees and annual training, including
<br /> refresher courses, shall include safety procedures for the event of a release or threatened release of a hazardous
<br /> material, including, but not limited to, familiarity with the facility's emergency response plans and procedures. These
<br /> training programs may take into consideration of each employee. Immediately prepare or update the facility's
<br /> training program , log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/,
<br /> upload the correct or updated information , and submit to the EHD for approval.
