The following is an itemized list of hazardous materials violations that have not been <br /> addressed for UNION POINT MARINA BAR & GRILL INC as of July24, 2017. <br /> Open violations from June 22, 2017 inspection <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually for the years 2014, 2015, 2016 and 2017. The hazardous materials inventory shall be <br /> submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. <br /> Immediately log into the CERS at http://cers.calepa. ca.gov/, enter the correct or updated information , and submit to <br /> the EHD for approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> Violation #12 = Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section . Immediately make arrangements to have the business <br /> plan readily available for review. <br /> Page 2 of 2 <br />
