SAN '�d DAQ II II Environmental Health Department <br /> c0uNT <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> YOSEMITE FOODS 4221 E MARIPOSA RD, STOCKTON December 17, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Facility could not provide training records during the inspection on evacuation plans and <br /> procedures for all the employees. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of <br /> three years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan(HMBP) inspection on 12/17/2021. There were six (6)violations <br /> during the inspection. An inspection checklist was emailed to the facility at the time of the inspection. A complete <br /> inspection report was generated today and will replace the initial checklist received on 12/17/21. A copy of the <br /> complete inspection report is emailed to Eloy Lombrana: Eloy@Yosemitefoods.com <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 1/19/2022. <br /> Compliance Notes: <br /> A- Correct the following elements by next submittal: <br /> 1- Facility stores 330 gallons of coil guard and only reported 220 gallons. <br /> 2- Facility reported 20 gallons of DEF, but is no longer using/storing DEF. <br /> 3- Facility stored a total of 660 gallons of oil inside the shop and only reported 440 gallons total of oil. <br /> 4- Facility is testing the use of two new chemicals (polymer stored in a 300 gallons mixture tank and ferric <br /> chloride stored in two 275 gallons totes). These chemicals have been onsite for two weeks and stored outside <br /> near the daft unit and inside the building by the daft unit. These chemicals are not reported mto CERS.
