Please find attached the five year permit review form, a new <br /> front page of the permit and letter to the operator asking that sections of the Joint <br /> Technical Document(JTD)be updated. The EHD made the finding that no significant <br /> changes had occurred. <br /> The facility operator had submitted a revised JTD on October 27, 2010. In addition to <br /> amendment to the JTD,the EHD received a Solid Waste Facility Permit(SWFP) <br /> application form.
