The following is an itemized list of hazardous waste violations that have not been <br />addressed for Fast Lane Central Valley as of May01, 2018. <br />Open violations from June 17, 2016 inspection <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br />of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br />web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br />Immediately reactivate your state ID number and submit evidence to the EHD. <br />Violation #117 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on June 29, 2015 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by July 30, 2015. This <br />information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br />Note -2 violations, lack of emergency coordinator and contaminated container were observed as having been <br />corrected today on site. The other 2 violations, failure to maintain adequate aisle space and inactive state ID <br />number remain open. <br />Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br />Saturated absorbent used to soak up what appeared to be fuel near dispenser 7/8 on the auto side and throughout <br />the facility near the dispensers was seen on the ground. A 5g bucket full of saturated absorbent, which was <br />determined to be hazardous waste at the time of inspection had a crack in the bottom and oily staining under and <br />around it. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of <br />hazardous waste to air, soil, or surface water which could threaten human health or the environment Immediately <br />pick up the saturated absorbent and manage according to Title 22 hazardous waste regulations. Submit a <br />statement and supporting documentation explaining how this waste was managed. <br />Violation #304 - Failed to maintain adequate aisle space for unobstructed response to emergencies. <br />2 hazardous waste drums are stored in the dumpster area behind 2 dumpsters. The area has trash littered <br />throughout it. To get to the hazardous waste containers one must squeeze between the two dumpsters or pull them <br />out. Adequate aisle space shall be maintained to allow the unobstructed movement of personnel, fire protection, <br />spill control equipment, and decontamination equipment to all areas of facility operation in an emergency. <br />Immediately maintain aisle space to allow the unobstructed movement of personnel, fire protection, spill control <br />equipment, and decontamination equipment to any area of facility operation in an emergency. <br />Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br />One -30g drum was seen without the contents marked. All hazardous waste containers shall be marked with the <br />following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. <br />This violation was corrected on site. <br />Open violations from June 29, 2015 inspection <br />Violation #101 - Managed a hazardous waste without a valid EPA ID number. <br />This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or <br />offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate <br />your EPA ID number and submit evidence to the EHD. <br />Page 1 of 2 <br />
