The following is an itemized list of hazardous waste violations that have not been <br />addressed for Fast Lane Central Valley as of May01, 2018. <br />Open violations from June 29, 2015 inspection <br />Violation #104 - No modified contingency plan. <br />An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br />emergency coordinator on site or on call to coordinate emergency response measures, and the following <br />information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br />extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br />unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br />information by a phone. A form is provided that can be used for this purpose. <br />Violation #304 - Failed to maintain adequate aisle space for unobstructed response to emergencies. <br />The drums for hazardous waste were stored behind two dumpsters with a narrow passage in between the two, in a <br />trash enclosure. The trash enclosure had inches of trash on the ground in the enclosed area. Adequate aisle <br />space shall be maintained to allow the unobstructed movement of personnel, fire protection, spill control equipment, <br />and decontamination equipment to all areas of facility operation in an emergency. Immediately maintain aisle space <br />to allow the unobstructed movement of personnel, fire protection, spill control equipment, and decontamination <br />equipment to any area of facility operation in an emergency. <br />Violation #406 - Contaminated container not marked with date emptied or managed within one year of being <br />emptied. <br />2-55g empty drums were seen in the hazardous waste storage area. Containers larger than five gallons that <br />previously held a hazardous material shall be marked with the date they were emptied and managed within one year <br />of being emptied.
