Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00053629 Site Location: 0 HOWLAND RD <br />Activity Summary <br />Activity Date Recorded by <br />04/05/2021 8:55 AM Received notification from Ed Lietch (LH Environmental) 916-847-1763 of MURO <br />approximately 100-gallon diesel fuel spill that occurred on 4/2/21. The source of <br />the spill was from an impacted saddle tank to a semi truck (Mike Lowrie Trucking). <br />The cause of damage to the saddle tank is unknown. No waterways impacted. No <br />evacuations or exposures. <br />The area of soil impacted was approximately 15 x 30 feet. <br />Ed reported that he was trying to call 209-468-0343 over the weekend to report the <br />spill, but could not get through. Let him know he would need to notify CAL OES <br />and provided the number. <br />Ed Lietch is the Responsible party (RP) contact. <br />RP info: <br />Mike Lowrie Trucking <br />7134 Tremont Rd. <br />Dixon, CA 95620 <br />Ancon was contracted for cleanup. USA Dig notification was initiated by Ancon on <br />4/5/21. <br />9:50 AM Headed to the spill site. Arrived at 10:18 AM. The spill site was directly <br />across the street from Simplot, 16777 Howland Rd, Lathrop approximately 12 feet <br />west of the Union Pacific railroad track. <br />Observed absorbent over the spill area. The USA dig markings covered <br />approximately 15 x 30 feet in area. Observed a separate diesel spill area <br />approximately 2 x 5 feet outside of the USA dig markings. A single trail of diesel <br />staining approximately 6 inches wide was visible from the road to the spill area. <br />Updated Lead Senior Garrett Backus, then returned to EHD. <br />Called Mike Algots (Hazmat Manager at Union Pacific Railroad) 916-580-9999 to <br />notify of spill and to inquire whether the spill was on Union Pacific property. Mike <br />was not aware of the spill and requested coordinates, and he will look into it and <br />let me know. <br />Emailed Mike Algots coordinates: 37.808664, -121.275299 to the approximate <br />location of spill at: mjalgots@up.com. <br />Generated site map. <br />Updated Responsible party and date notified to the complaint. <br />Received a call from Mike Algots reporting that the spill location was on Union <br />Pacific right-of-way. I provided responsible party contact information. Mike will <br />generate an email to the responsible party's contact, UPRR Track Manager: <br />Richardo Casas, and a copy to me to coordinate remediation of the area. The <br />track will have to be flagged during remediation since spill is close in proximity. <br />Updated complaint information form: nature of complaint, property owner, and <br />APN number. <br />Completed Notice to Abate.
