Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00053629 Site Location: 0 HOWLAND RD <br />Activity Summary <br />Activity Date Recorded by <br />Received call forward from front desk. Union Pacific Railroad wanted to report the <br />100 - gallon spill. I let her know I have received the information. <br />Union Pacific Railroad Police called to ask if Responsible party of the 100 gallon <br />spill notified EHD. I let caller know we were notified this morning of the spill that <br />occurred on Friday, 4/2/21. <br />Called Ed Leitch to confirm receipt of Notice to Abate; Ed confirmed he received <br />the notice. Advised Ed of spill areas noted during my site visit. Ed plans to be out <br />there and will ensure all areas are addressed. He asked if I can email him photos <br />of the spill areas. Emailed him the photos. <br />Completed Prop 65 and submitted to Garrett Backus for review. <br />04/07/2021 12:24 PM Received a call from Ed Lietch letting me know that cleanup will take MURO <br />place tomorrow, April 8. I asked that he gives me a phone call about 45-minutes <br />prior to completion of cleanup. <br />04/08/2021 9:31 AM Received a call from Ed Lietch stating that they we ready for me to head MURO <br />out to the diesel spill cleanup. They were about 30 - 45 minutes from being done <br />with excavating. <br />10:00 AM Left EHD, headed to spill site. <br />10:20 AM Arrived at spill cleanup. Met with Ed Lietch (Mike Lowrie Trucking), <br />Julian [did not obtain last name] (Golder Associates, Consultant for UPRR), <br />Noland Ryan (Field Supervisor at Ancon Services). <br />Observed the excavation to be an area of 15 - 30 feet. The depth excavated <br />varied between 10 - 18 inches. Ancon utilized a Photoionization Detector (PID) <br />meter to measure levels of organic compounds in parts per million. One area in <br />the soil was darker. Ancon used PID meter in m presence to test the darker soil. It <br />measured 0.0 PPMs. One of the Ancon workers said it was moisture in the soil <br />beneath and kicked aside some soil in a different area within excavation, and it <br />was also darker. I was not detecting odor, and did not observe staining. Ancon <br />also excavated the trail of soil that was impacted from the semi truck as it leaked <br />towards the main spill area. The cleanup was adequate based on no diesel <br />staining, or odor. One 20 yard bin was filled with diesel-contaminated soil and to <br />remain on-site pending analytical testing for waste profile. <br />Witnessed confirmation sample by Ancon from center of excavation. <br />Ed Lietch reported that all analytical lab results and disposal records will be <br />submitted to EHD in a report within 30 days. I asked him to provide copies of <br />analyticals sooner that that. <br />Left site. <br />11:05 AM Arrived at EHD <br />04/13/2021 Received an email from Nissinn, Melissa [EHD] <mnissim@sjgov.org>, the email <br />contained a forwarded email from Bencomo, Isabella@Waterboards <br /><Isabella.Bencomo@Waterboards.ca.gov >, who is requesting a copy once the <br />compliant is abated. Forwarded the information to C. Muro (EHD inspector). Sent <br />an acknowledgment email to M. Nissim. <br />MANZO <br />04/20/2021 Received staatus update from LMH Environmental.
