Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COVOMOG3H Site Location: 2121 N ZUCKERMAN RD <br />Activity Summary <br />Activity Date Recorded by <br />06/09/2020 Called and spoke to Ralph Roberts of PG&E. Scheduled to meet Ralph Roberts at FLORIDO <br />the spill location today at 9:15 AM. Prepared for inspection. <br />Arrived on-site and met with Ralph Roberts. Observed the secondary containment <br />holding the release gylcol. No over spill or leakage from the secondary <br />containment was observed at the time of inspection. Observed the valve from <br />which the release occurred was closed. No ongoing release was observed. Took <br />photos. Ralph stated he did not yet whether the waste gylcol would be managed <br />as hazardous or non-hazardous. Told Ralph to keep Claudia update on when the <br />clean-up would occur in case EHD would like to come back out to verify, and on <br />the management of the waste. Left site for EHD. <br />Generated photo log and emailed it to Claudia Muro, EHS. Updated Claudia and <br />Elena Manzo on my observations. <br />06/09/2020 8:00 AM Received update via email from Ralph Roberts sent on June 8, 2020 at <br />5:31 PM. <br />PG&E verified the amount released to be 1300 gals of used Triethylene glycol to <br />the secondary containment. They are arranging for off-haul with PSC. They <br />notified CA OES before the actual volume was determined. Cal OES number is <br />20-3037. They will provide update to Cal OES. <br />MURO <br />06/09/2020 8:05 AM Forwarded Ralph Roberts' (PG&E) email to Elianna Florido, (EHD) and MURO <br />Elena Manzo, (Lead Senior, EHD) stating that PG&E has verified the amount of <br />triethylene glycol to be approximately 1300 gallons. <br />10:53 AM Received update from Elianna Florido. She emailed me her findings <br />from the PG&E site visit and a photo log. Elianna Florido advised Ralph Roberts to <br />contact me with updates. <br />06/09/2020 Generated Notice to Abate and emailed it to Ralph Roberts, (PG&E) at MURO <br />RPRb@pge.com. <br />06/10/2020 Called Ralph Roberts (PG&E) for status on the cleanup of the secondary MURO <br />containment. Ralph reported that PSC was onsite. They will remove all triethylene <br />glycol from the secondary containment, sample it, and place in a frac tank pending <br />disposal based on the triethylene glycol sample results. <br />06/10/2020 Updated Elena Manzo (EHD) on the status. She has asked that I go to verify at the MURO <br />end of cleanup. <br />I called Elianna Florido (EHD) to ask if she can provide directions to the facility, <br />since I have never been there. We also discussed the need to wear a fire <br />retardant suit. <br />06/10/2020 Went to 2121 N Zuckerman Rd (McDonald Island) PG&E Turner Cut Station to MURO <br />verify cleanup of the secondary containment. Met with Ralph Roberts, (PG&E). <br />The triethylene glycol liquid was pumped out by PSC contractors. <br />06/29/2020 Received analytical report from Ralph Roberts (PG&E).
