San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CVS Pharmacy#5403 1175 W Lathrop Rd, Manteca September 06, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16(a-c)Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> Although April Barnes,the store manager,provided proof of current training for each employee,the following <br /> observations made at the time of inspection indicate that the training provided to employees who handle hazardous <br /> waste is not adequate: <br /> -In the hazardous waste(HW)storage area: <br /> -A HW Oxidizers tote with accumulation start date 8/26/18, lacked the physical state on the HW label and did not <br /> indicate that the tote contained Hair Bleach Products(see IMG 1614).April corrected the label at the time of <br /> inspection(IMG 1615-1616). <br /> -A HW Flammables tote with accumulation start date 8/6/18, contained a bottle of Christian Brothers Brandy(IMG <br /> 1618), Lysol Disinfectant Spray(IMG 1619), Aussie hair spray(IMG 1619), Garnier Fructis Volumizing Mousse(IMG <br /> 1619), and CVS Health Maximum Toothache&Oral Pain Reliever(IMG 1620-1621). However,the HW label on the <br /> tote did not indicate these items as contents in the tote(IMG 1617). <br /> - A HW Toxics tote with accumulation start date 8/26/18, contained Neutrogena Facial Moisturizer and Maybelline <br /> Great Lash Mascara(IMG 1625), however the HW label on the tote did not indicate these items as contents in the <br /> tote(IMG 1626). April corrected the label at the time of inspection(IMG 1627). <br /> -A second HW Toxics tote with accumulation start date 8/26/18, contained CVS Health Nicotine Transdermal <br /> System Patches and Cetaphil Lotion(IMG 1632-1633), however the HW label on the tote did not indicate these <br /> items as contents in the tote(IMG 1634).April corrected the label at the time of inspection(IMG 1635). <br /> -A House To Home oscillating pedestal fan was found in an open box, labeled as"UW'and without an <br /> accumulation start date, on a shelf adjacent the HW storage area(IMG 1636-1637). <br /> -When asked how often damages and returns are processed,April stated 2-3 times a week.An open red bin <br /> labeled as"Damages,"located adjacent the HW storage area, contained the following (IMG 1669): <br /> -A CVS Insect Repellent scanned as Flammable HW(IMG 1551-1652). <br /> -A bottle of lemon-scented liquid was found leaking in a StrongPak bag and scanned as Toxic HW(IMG <br /> 1653-1654). <br /> -A bag of CVS Epsom Salt was found leaking in a bag and scanned as Toxic HW(IMG 1655-1656). <br /> -A CVS Health Sinus Wash System was found damaged and scanned as Toxic HW(IMG 1657-1658). <br /> -A CVS Health Lice Killing Shampoo was found damaged and scanned as Toxic HW(IMG 1663-1664). <br /> -A RID Lice Killing Shampoo was found damaged and scanned as Flammable HW(IMG 1665-1666). <br /> -A bottle of Organic Rose Otto Skin Toner was found partially empty and scanned as Toxic HW(IMG 1667-1668). <br /> All of the waste listed above were identified as waste by April Barnes. <br /> -The following was found in an open unlabeled grey tote(IMG 1674-1675)), located adjacent to the HW storage <br /> area: <br /> -A bottle of Dawn Ultra dish soap, partially empty and scanned as Toxic HW(IMG 1670-1671). <br /> -A bottle of Axe Body Wash, partially empty and scanned as Toxic HW(IMG 1672-1673). <br /> All of the items listed above were identified as waste by April Barnes. <br /> -The following was found in an open unlabeled grey tote that was beneath a brown cardboard box(IMG <br /> 1683-1684), located adjacent to the HW storage area: <br /> -An open package of CVS Health Oral Anesthetic Liquid scanned as Flammable HW(IMG 1677-1678). <br /> -Three bottles of Babyganics Mineral-Based Sunscreen scanned as Toxic HW(IMG 1679-1680). <br /> -Two damaged packages of Nature's Bounty B-12 Sublingual Liquid scanned as Toxic HW(IMG 1681-1682). <br /> All of the items listed above were identified as waste by April Barnes. <br /> FA0019897 PR0531041 SCO01 09/06/2018 <br /> EHD 22-03 Rev.09/22/16 Page 7 of 16 RCRA Large Quantity Hazardous Waste Generator OIR <br />
