San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.orci/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CVS Pharmacy#5403 1 1175 W Lathrop Rd, Manteca September 06, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> -The following was found in an open unlabeled blue container in the pharmacy(IMG 1685): <br /> -Two StrongPak bags containing loose pills <br /> -One StrongPak bag containing a vial of loose pills <br /> -A white bottle of pills <br /> -Accu-Chek Softclix 100 Lancets <br /> All of the items listed above were identified as waste by the Staff Pharmacist, Lakshmi Deepa Hanumandhu. <br /> Lakshmi also stated that items placed in this blue bin is processed every Sunday. <br /> -The following was found in an open unlabeled grey tote beneath a register at the front of the store(IMG 1706): <br /> -CVS Health Saline Nasal Spray scanned as Toxic HW(IMG 1687-1688). <br /> -Neutrogena Anti-Aging Perfector scanned as Toxic HW(IMG 1689-1690). <br /> -CVS Health Sterile Saline Nasal Mist scanned as Flammable HW(IMG 1691-1692). <br /> -Elf Waterfproof Volumizing Mascara scanned as Toxic HW(IMG 1693-1694). <br /> -Revlon Age Defying Foundation scanned as Toxic HW(IMG 1695-1696). <br /> -CVS Health Dissolving Whitening Treatment scanned as Oxidizer HW(IMG 1697-1698). <br /> -An open Glazed Lip Pain scanned as Toxic HW(IMG 1699-1701). <br /> -Maybelline Dream Liquid Mousse scanned as Flammable HW(IMG 1702-1703). <br /> -Maybelline Tattoo Studio Brow Pen scanned as Flammable HW(IMG 1704-1705). <br /> All of the items listed above were identified as waste by April Barnes. <br /> Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual <br /> review of the initial training received. At a minimum,the training program shall be designed to ensure that facility <br /> personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures, <br /> emergency equipment, and emergency systems, including where applicable: <br /> 1. procedures for using, inspecting, repairing,and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents; and <br /> 6. shutdown of operations. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately <br /> provide employees with initial or refresher hazardous waste training.
