Per our conversation...
I've received the permit for replacement of the 87B tank drop tube. According to the application, the replacement took
place on 10/10/22. The work order included with the application indicates an order date of 9/30/22 and a replacement
date of 10/10/22 by Jones Covey. Below are the items we discussed:
1. Replacement of overfill prevention equipment requires a permit be obtained and approved by EHD prior to work
being conducted. Since the work has already been completed, a Failure to Obtain a Permit penalty applies (an
additional $468). The permit application cannot be processed until all fees have been submitted.
2. The overfill was tested on 8/30/22 with an EHD inspector present. The work order indicates a replacement was
ordered on or before 9/30/22 and states "T2 has an overfill flapper that is faulty". How was it determined that
the flapper was faulty?
3. The work order lists an Order Quantity of"2 each". Were two overfill drop tubes replaced?
4. Why was the work conducted without a permit?
5. Why was EHD not notified of failed overfill?
I've attached a copy of the work order and our Service Request Guidance document. The permit will be processed as
soon as full payment is received and is not held up by the questions listed here. Please let me know if you have any
other questions.
Thank you,
Stacy Rivera
2
