Immediately begin using a registered <br /> hazardous waste hauler to transport any generated hazardous wastes. <br /> This is a Class I violation. <br /> 203 HSC 25189.5(c)Transported or caused transportation of hazardous waste to an unauthorized point. <br /> According to the pharmacy manager,Shuchi Gupta,expired Warfarin and Coumadin are sent to Genco.Genco <br /> does not have a permit from the Department of Toxic Substance control(DTSC)to received hazardous waste. <br /> No person shall transport or cause transportation of hazardous waste to a facility which does not have a permit from <br /> DTSC.Immediately cease the transportation of hazardous waste to unauthorized locations. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2)Facility not maintained to minimize the release of a hazardous waste. <br /> Eight containers of Oxi Clean laundry detergent,one leaking and another with a broken cap,were found under an <br /> empty grey tote in the mop sink(see photo: IMG 1338-1341). <br /> Facilities shall be maintained and operated to minimize the possibility of a fire,explosion,or release of hazardous <br /> waste to air,soil, or surface water which could threaten human health or the environment. Immediately clean the <br /> leakage and manage the clean-up debris and eight containers of Oxi Clean laundry detergent according to Title 22 <br /> hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was <br /> managed. <br /> This is a Class II violation. <br /> 302 CCR 66262.34(d)(2)Failed to maintain required emergency equipment. <br /> An"Out Of Order"sign was observed for the eye wash station,located above the mop sink near the hazardous <br /> waste storage area(see photo: IMG 1369).At the time of inspection,the store's stock clerk receiver, Ray Koker, <br /> tested the eye wash station and water came out from the station. <br /> All facilities shall be equipped with an internal communication or alarm system, a device capable of calling outside <br /> emergency help,spill control equipment,and decontamination equipment at all times.
