Sally Beauty Supply #10530, 4339 E Morada Ln Stockton, CA Record ID PR0541186 <br />12/27/2018 <br />PHOTO # 1 <br />Elianna Florido, REHS Page 1 of 11 <br />When asked what type of training she and the rest of the store employees have re ceived, Zulema Ojeda, the store manager, stated she was told by her manager to watch <br />the Sally Beauty Hazardous Waste Training DVD at the beginning of the year, which she and the other employees watched at the beginning of 2018 for the first time. <br />Zulema stated she was told by her manager that the DVD is to be viewed by all the employees at least once each year. Zulema stated that the DVD has been in the onsite <br />hazardous waste binder since she started working at this facility about 2-3 years ago. A Waste Management Training DVD Sign Off sheet was observed on site, which was <br />signed off by Zulema Ojeda, Guadalupe Morales, Leslie Sanchez, and Priscilla Williams on 1/8/18. Zulema did not have any other employee training records at the time of <br />inspection. <br />
