On all Designated Operator reports observed , starting 11 /8/2021 , a W1 <br /> WPLLD communication alarm has been noted as in alarm but has not yet been addressed . The alarm history observed during the inspection shows th <br /> initial W1 WPLLD communication alarm on 10/22/2021 . <br /> During the 5/26/2022 inspection , dispensers 1 /2 , 11 /12 , 13/14 , 15/16 , 17/18 , 19/20 , and 21 /22 were observed with piping located outside of the unde <br /> dispenser containment (UDC) that would not allow leaks to flow back into the UDCs . The piping located outside of UDCs were initially observed o <br /> 10/30/2020 on permit number SR0082297 inspection for dispenser and conversion frame installation . The piping located outside of the UDCs have bee <br /> cited as violations during the 5/26/2021 and 5/26/2022 inspection reports . On 10/6/2021 Bravo Retrofit Plates were installed to direct leaks from th <br /> piping into the UDCs but were installed by a technician not certified by the manufacturer and these Bravo Retrofit Plates were not a manufacture <br /> approved use for the plates . Per the manufacturer, this facility should install conversion frames with trays that have a slope to allow leaks into the UDC . <br /> - Corrective action statements in response to inspections 5/30/2019 , 5/18/2020 and 5/26/2021 were not submitted to EHD .
