OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the 12/30/2021 designated operator inspection report and failed to check that they were responded to appropriately. The missing alarms include: -T2 91 Overfill Alarm on 12/29/2021. REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. --------------------------------------------------------------- --------------------------------------------------------------- OBSERVATION: The designated operator failed to check and note the correct test dates for the monitoring system certification and spill container testing on the 3/26/2021 designated operator inspection report.The incorrect dates were noted as 1/27/2020. The correct date for the monitoring system certification and spill container testing is 1/26/2021. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. --------------------------------------------------------------- --------------------------------------------------------------- OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 1/28/2021, next inspection performed on 3/26/2021 (57 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. --------------------------------------------------------------- --------------------------------------------------------------- OBSERVATION: The owner or operator did not provide a description of each corrective action taken or to be taken, acknowledging the identified compliance issues, on the designated operator inspection reports: -The 3/26/2021,4/23/21, 11/5/21, 12/3/21, and 12/30/21 designated operator reports were missing the owner/operator description of corrective actions. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report.
