Within 72 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention <br /> equipment inspection on the designated operator inspection reports. The incorrect date was listed as 11/14/2018 <br /> on the designated operator report 12/30/2021, 1/28/2022, and 9/8/2022. The correct date for the overfill prevention <br /> equipment is 11/17/2021. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the diesel STP sump and 3-4 UDC. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately 1 cup of liquid from the diesel STP sump <br /> and approximately 2 oz of liquid was removed from the 3-4 UDC.
