OBSERVATION:A new designated operator, Rolando Urbina, began conducting inspections on 6/1/2021 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE: The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form"no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a Class II violation. 117 CCR 2637(e), 2637(f)Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. OBSERVATION: Secondary containment testing was performed on 1/29/2021.A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). REGULATION GUIDANCE:A copy of the'Secondary Containment Testing Report Form'must be submitted within 30 days of the test. CORRECTIVE ACTION: Immediately provide this report to the EHD. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The UST training form on site was on an incorrect form. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: The Designated Operator(Rolando Urbina)provided training during the inspection and the Facility Employee Training Certificate was corrected on site. No further action required.
