<br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste
<br /> handling or the training provided has been inadequate.
<br /> The following observations were made in the store's back storage room at the time of inspection:
<br /> -The hazardous waste (HW)totes for Corrosive Bases;Aerosols, Flammables, and Combustibles; Corrosive Acids;
<br /> and Toxic and State Regulated Waste were found open with cracked and broken lids(see photos: IMG 2524, 2525,
<br /> 2614, and 2579).
<br /> -The most recent Weekly Inspection Log found on site was completed by Patricia Brewer, the store manager, and
<br /> was dated 1/6/2017 (IMG 2546).
<br /> -All six of the hazardous waste totes observed contained HW, but lacked accumulation start dates and the name of
<br /> the generator(IMG 2511, 2549, 2559, 2580, 2601, 2608, and 2617).There was one tote for Corrosive Bases, one
<br /> for Aerosols, Flammables, and Combustibles, one for Corrosive Acids, one for Oxidizers, and two for Toxic and
<br /> State Regulated Waste.
<br /> -A Clairol Professional Creme Hair Lightener was found in the Corrosive Acids HW tote (IMG 2549-2552).
<br /> However, according to a label on the Oxidizers HW tote, creme hair lighteners are classified as oxidizers. (IMG
<br /> 2567-2568).
<br /> -Several tubes of Ion Semi-Permanent creme hair color were found in the same bag with Wella Permanent gel hair
<br /> color(IMG 2561-2562)and in another bag with Loreal Technique Permanent Highlight Cream (IMG 2570-2572),
<br /> inside the Corrosive Bases HW tote(IMG 2558-2563). However, according to a label on the Toxic&State
<br /> Regulated HW tote, "semi hair color"is classified as a toxic and state regulated HW.Also, according to Sally Beauty
<br /> Holding's"Guidelines for Accumulating Hazardous Waste"poster, posted on a wall in the HW storage area, only
<br /> one waste item should be placed into a bag (IMG 2557).
<br /> -A bottle of AGE Beautiful Gentle 20-volume Creme Developer was found in the Corrosive Bases HW tote (IMG
<br /> 2558-2560, 2565-2566)and another bottle of the same developer was found in the Aerosols, Flammables&
<br /> Combustibles HW tote(IMG 2579-2581, 2586-2587), although according to a label on the Oxidizers HW tote, "all
<br /> volumes of developer"are classified as oxidizers(IMG 2567-2568).
<br /> -The HW items listed below were found in HW totes with labels that did not include these items as contents of the
<br /> totes, nor were the items on the Sally Beauty Holdings Inc. "Hazardous Material Management"poster.When asked
<br /> how a waste determination is made for such items,Tina Flores,the assistant store manager, stated she either
<br /> asked the store manager, Patricia Brewer, or she would guess which HW tote the item should be placed in. Tina did
<br /> not appear familiar with the Sally Beauty Hot Line number listed on the"Hazardous Material Management"poster to
<br /> call if questions came up.Also, Tina did not know about the Sally Beauty Application,which provides item Safety
<br /> Data Sheets(SDS)to make waste determinations, until she called her District Manager, Hillary Mcgahey.At this
<br /> point,Tina still was not clear on information needed to help make a waste determination from the SDS.Towards the
<br /> end of the inspection she told us there is a new poster to help identify waste streams that apparently was sent to the
<br /> store, but they had not received.
<br /> -Two containers of Gigi Hemp Wax, one of which was leaking and without absorbent,were found in the Corrosive
<br /> Acids HW tote(IMG 2549-2550, 2554-2555).According to Sally Beauty Holding's"Guidelines for Accumulating
<br /> Hazardous Waste" poster, absorbent must be placed in the bag with a leaking container(IMG 2557).
<br /> -A container of GIG Sensitive Tweezeless Wax was found in the Oxidizers HW Tote(IMG 2601-2604).
