San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SALLY BEAUTY SUPPLY 1 1163 S MAIN ST, MANTECA December 28, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> The following observations were made in the store's back storage room at the time of inspection: <br /> -An Oxidizer HW tote contained an ASP Aspire Bonding Acrylic Liquid, labeled as"flammable"(IMG 3364-3368). <br /> Although, according to the Sally Beauty Holdings Inc. "Hazardous Material Management"poster(The Poster)(IMG <br /> 3360)and the label on the Flammables HW tote(IMG 3405),flammable liquids shall be placed in the Flammables <br /> HW tote. Gianna Garcia,the assistant store manager, placed the item into the Flammables HW tote at the time of <br /> inspection. <br /> -An Oxidizer HW tote contained a bag which contained a tube of Gigi#1 Bleaching Cream and a tube of#2 Cream <br /> Activator.According the Safety Data Sheet(SDS), the cream activator contains ammonium hydroxide(IMG <br /> 3374-3375).According to The Poster, items containing ammonium hydroxide shall be placed in the Corrosive Bases <br /> HW tote.According to the Sally Beauty Holdings, Inc. Guidelines For Accumulating Hazardous Waste poster, and <br /> the Hazardous Waste Training Guide, only one waste type/item shall be placed in any one bag (IMG 3360, 3437). <br /> Gianna placed the cream activator into the Corrosive Bases HW tote at the time of inspection. <br /> -A Corrosive Acids HW tote contained bottles of Zotos Professional Quantum hair waving lotion, neutralizer, and <br /> activator(IMG 3401-3404).According to the ingredient list on the product box, the neutralizer contains hydrogen <br /> peroxide.According to The Poster, items containing hydrogen peroxide shall be placed in the Oxidizers HW tote. <br /> According to the Sally Beauty Holdings, Inc. Guidelines For Accumulating Hazardous Waste poster, and the <br /> Hazardous Waste Training Guide, only one waste type/item shall be placed in any one bag. Gianna placed the <br /> neutralizer into the Oxidizers HW tote at the time of inspection. <br /> -A Flammable HW tote contained a bottle of Hawaiian Silky Miracle Worker 14-in-1,which according to the SDS is <br /> "NOT Flammable"(IMG 3405, 3407-3409). Gianna placed the item into the State Toxic HW tote at the time of <br /> inspection. <br /> -Hazardous waste containers are not inspected weekly. The following hazardous waste(HW)containers were <br /> observed with broken/cracked lids in the facility's HW storage area: <br /> -An Oxidizers HW tote(IMG 3376-3378). <br /> -A State Toxic HW tote (IMG 3384-3387). <br /> -A Corrosive Bases HW tote (IMG 3393-3397). <br /> -A Corrosive Acids HW tote(IMG 3398-3401). <br /> -A Flammables HW tote(IMG 3405-3406). <br /> The most recent Weekly Inspection Log was completed on September 24, 2018 by Sandra Nunez,the store <br /> manager, according to Gianna (IMG 3439). Gianna did not know why the Weekly Inspection Logs stopped at <br /> September 24th. <br /> The following HW containers were found lacking the full name of generator: <br /> -An Oxidizer HW tote with accumulation start date 10/26/18(IMG 3364). <br /> -Twp State Toxic HW totes with accumulation start dates 10/26/18 (IMG 3384 and 3411). <br /> -A Corrosive Bases HW tote with accumulation start date 10/26/18(IMG 3393). <br /> -A Corrosive Acids HW tote with accumulation start date 10/26/18(IMG 3398). <br /> -A Flammables HW tote with accumulation start date 10/26/18(IMG 3405). <br /> -A Beauty AG Root Touch Up temporary hair color aerosol can, labeled as"Damaged 4/27/17"and "Extremely <br /> Flammable"was found on the floor behind the stacked Universal Waste totes adjacent the facility's hazardous <br /> waste storage area(IMG 3430-3432).
