Gianna placed the item into a <br /> Universal Waste tote. <br /> -An LED Nail Lamp,which contained a plug-in cord and was labeled with SKU number 156001 (IMG 3420-3422). <br /> Gianna place the item into a Universal Waste tote. <br /> -A One N'Only Argan Heat Ceramic Curling Iron,which contained a bottle of One N'Only Argan Oil Treatment <br /> (IMG 3423, 3428-3429). Gianna placed the curling iron into a Universal Waste tote and the Argan Oil Treatment into <br /> a Flammables HW tote. <br /> -A bottle of Ardell Professional DUO Individual Lash Adhesive,which was labeled with a flammable symbol (IMG <br /> 3424-3427). Gianna placed the item into a Flammables HW tote. <br /> -When asked what the store's process is for cleaning up items that spill on the floor, Rebecca Roth, a sales <br /> associate, stated employees use kitty litter to clean up spills and throw the used kitty litter in the trash. <br /> -When asked how waste determinations are made, Gianna stated if an item is not indicated on The Poster or the <br /> HW tote labels, she asks the Store Manager, Sandra Nunez, to determine which bin an item should be placed in. <br /> Gianna stated she has never used the Sally Beauty Hot Line number listed on The Poster, nor has she used the <br /> Sally Beauty Safety Data Sheet(SDS)application on the store's Ipad, if questions came up regarding hazardous <br /> waste categorization. <br /> -Gianna stated she has not watched the Store Hazardous Waste Training Video, nor the How To Categorize <br /> Products-California Video from the Athena application on the store's Ipad (IMG 3362-3363). Gianna stated she did <br /> not know if the store manager, Sandra Nunez, has watched the videos on the Athena application. <br /> -The Employee Hazardous Waste Training Log observed at the time of inspection had the signatures of <br /> employees,which were dated from 2017. Gianna stated this training log indicated which employees watched the <br /> Sally Beauty Holdings, Inc. Hazardous Waste Training DVD and reviewed the Hazardous Waste Training Guide <br /> (IMG 3436, 3437).
