The following facility's procedure for containing spills on the Emergency <br /> Contingency Plan section D was not implemented: <br /> #8 Stop Processes and/or operation <br /> #12 Notify and evacuate persons in all threatened and/or impacted areas" <br /> #15 Remove containers and/or isolate areas <br /> REGULATION GUIDANCE: A Hazardous Materials Business Plan (HMBP)shall be established and implemented <br /> for emergency response to a release or threatened release of a hazardous material when storing hazardous <br /> materials at or above the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for solids, or <br /> 200 cubic feet for compressed gas, at standard temperature and pressure. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD./Submit a <br /> statement/documentations to SJC EHD demonstrating that the HMBP has been established and implemented. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. There was no updated site map for the location of storm and sewer <br /> drains and inlet ponds. Per Angela Luna,the construction began May 1st, 2021.The three retention ponds in the <br /> northeast side of the facility was reduced to one. The retention pond on the west side was removed and the two <br /> ponds on the east side was combined into one large retention pond.A parking structure is build over the retention <br /> pond on the west side. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> FA0023065 PR0541209 SCO05 03/24/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
