San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sogov.org/ehd <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />1 <br />Date: <br />SALLY BEAUTY SUPPLY <br />1240 W ELEVENTH St, TRACY <br />January 07, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />106 <br />CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />The following observations were made in the store's back storage room at the time of inspection: <br />- A blank Waste Management Training DVD Sign Off sheet (see photo IMG 3457) was observed inside the Sally <br />Beauty Holdings, Inc. Hazardous Waste Training Guide binder (IMG 3449). According to the store manager, <br />Whitney Flores, she has seen only part of the Sally Beauty Holdings, Inc. Hazardous Waste Training DVD (IMG <br />3451), and she is not sure if the assistant store manager has watched the DVD. When asked if the other employees <br />have seen the DVD, Whitney stated that the other employees have not watched the DVD, since she thought that <br />only the store manager and assistant store manager are allowed to place damaged/returned items into the <br />hazardous waste (HW) totes. <br />- When asked who places damaged/returned items into the HW totes, Whitney stated herself and the assistant <br />store manager are the two employees that primarily place items into the HW totes, however, if an other employee <br />has worked for Sally Beauty for a long time, that employee may also place items into the totes. Whitney stated since <br />the store often replaces employees, the new employees will leave damaged/returned items behind the front register <br />for either herself or the assistant store manager to place into the correct HW totes, daily. Whitney stated she is at <br />work every day, except Sunday. <br />- The most recent Weekly Inspection Log found in the HW training guide binder was dated July 19, 2017 and was <br />completed by Whitney Flores (IMG 3458). When asked why the logs stopped on that date, Whitney stated she <br />hasn't had time to keep up logging the weekly inspections. <br />- When asked if she has seen the hazardous waste training videos on the Athena application on the store's Ipad, <br />Whitney stated she has not watched the videos. When trying to access the videos on the Athena application, <br />Whitney did not readily know where the videos were located. Once Whitney found the videos (IMG 3549-3550), she <br />stated she will be sure to watch the videos. <br />- When asked what she does when she has questions about which HW tote an item should be placed in, Whitney <br />stated when she cannot determine which tote an item should be placed in based on the Sally Beauty Holdings, Inc. <br />Hazardous Waste Management poster (The Poster) (IMG 3469) and the HW Information Guide (IMG3467-3468) <br />[both of these documents were taped onto the top of the Oxidizers HW tote (IMG 3465)], she will call other Sally <br />Beauty Supply stores to find out how another store manages the same item. Whitney did not appear familiar with <br />the Sally Beauty Safety Data Sheet (SDS) application on the store's Ipad (IMG 3495), nor the Sally Beauty Hotline <br />phone number to call if questions came up. The HW information Guide states, "Do not forget that the SDS app on <br />your iPad will give you all the information you need to help you separate your waste properly." When asked if she <br />uses the Sally Beauty Hotline phone number located on The Poster, Whitney stated she has not called the hotline <br />number. <br />- When asked what is done by employees when an item spills in the store, Whitney stated kitty litter, but primarily <br />Pinaud Clubman powder (IMG 3546, 3548), is applied to the spill and once the liquid is absorbed, the used <br />absorbent is swept up and disposed of in the trash. According to the Sally Beauty Holdings, Inc. Hazardous Waste <br />Training guide, used absorbent should be placed in a bag with the item that spilled, the name of the item should be <br />written on the bag, and the bag should be placed into the staging container (HW tote) (IMG 3456). <br />- The following HW totes were found with HW labels that did not include the full name of the generator: <br />FA0023431 PR0540944 SCO01 01/07/2019 <br />EHD 22-01 Rev. 09/22/16 Page 4 of 9 Small Quantity Hazardous Waste Generator OIR <br />
