OBSERVATION: UST Tank Information forms are not current in the California Environmental Reporting System (CERS). The tank form for tank-002 lists the contents as Diesel instead of Bio-diesel. REGULATION GUIDANCE:Any change of information must be updated in CERS within 30 days of the changes. CORRECTIVE ACTION: CERS was updated at the time of inspection. This is a repeat violation, Class II. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The owner/operator name didn't match the signature. -The reports note the containment sumps had issues, but no issues were listed in Section 3. -They incorrectly state the containment sumps had alarms requiring sump inspections. -Correct test dates are listed, but they incorrectly mark that testing was not done within the past 12 months. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated April 2021 had a blank CERS ID. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Ensure all future Facility Employee Training Certificates contain the accurate and complete information. This is a repeat violation, Class ll.
