San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CVS PHARMACY#16856 280 SPRECKELS AVE, MANTECA February 06, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 119 CCR 66265.16(d) Failed to maintain complete personnel training records. <br /> The past three years of hazardous waste training records for the following employees were not available at the time <br /> of inspection: <br /> -Renee Davis <br /> -Dalisa Ludwig <br /> -Myrna Leah Coronel <br /> -Sarah Kim <br /> Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. <br /> Ensure that employee training is properly documented. <br /> Submit to the EHD a copy of the past three years of hazardous waste training records for the employees mentioned <br /> above. <br /> This is a Class II violation. <br /> 201 HSC 25163(a)Transported or transferred hazardous waste to a transporter without valid registration. <br /> -According to Myrna Coronel,the pharmacy manager, pharmaceuticals that are sent to Genco are pulled in a 1 by <br /> 1 window,"which she explained to mean that pharmaceuticals that expired in the previous month, and the <br /> pharmaceuticals that expire in the current month and the next month, are pulled from the pharmacy shelves, in <br /> order to prevent expired or soon-to-be expired medicines to be dispensed to a patient.According to an Rx Genco <br /> Non-Controls Detail Reports dated 2/1/2018 and 4/4/2018 observed at the time of inspection,warfarin sodium <br /> tablets were sent to Genco(IMG 4125,4126). It could not be determined if the warfarin sodium tablets were expired <br /> when sent to Genco.According to Myrna, pharmaceuticals sent to Genco are mailed via UPS. <br /> -A tote(IMG 4115)was observed containing two labels-one label stated "No Hazardous Waste Pharmaceutical <br /> Reverse Processor Product(Genco)ONLY"and was adhered to the top of the tote(IMG 4113), and the second <br /> label stated"Non-Viable Hazardous Pharmaceutical Waste"with accumulation start date 1/7/19 and was adhered <br /> on one side of the tote(IMG 4114).According to Myrna, pharmaceuticals placed in this tote are sent to Genco every <br /> month via UPS. <br /> Transportation of hazardous waste shall be performed only a person that holds a valid registration issued by the <br /> Department of Toxic Substances Control to transport hazardous waste. <br /> Immediately begin using a registered hazardous waste hauler to transport any generated hazardous wastes. <br /> This is a Class I violation. <br /> FA0023487 PR0541025 SCO01 02/06/2019 <br /> EHD 22-03 Rev.09/22/16 Page 10 of 12 RCRA Large Quantity Hazardous Waste Generator OIR <br />
