It could not be determined if the warfarin sodium tablets were expired <br /> when sent to Genco. <br /> -A tote(IMG 4115)was observed containing two labels-one label stated "No Hazardous Waste Pharmaceutical <br /> Reverse Processor Product(Genco)ONLY"and was adhered to the top of the tote(IMG 4113), and the second <br /> label stated"Non-Viable Hazardous Pharmaceutical Waste"with accumulation start date 1/7/19 and was adhered <br /> on one side of the tote(IMG 4114).According to Myrna, pharmaceuticals placed in this tote are sent to Genco every <br /> month via UPS. <br /> No person shall transport or cause transportation of hazardous waste to a facility which does not have a permit from <br /> DTSC. <br /> Immediately cease the transportation of hazardous waste to unauthorized locations. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> The following was observed inside the CVS Pharmacy: <br /> -A tote was labeled Hazardous Waste Toxic Pharmaceuticals with accumulation start date 2/4/19 and contained <br /> only two used flu vaccine vials, but did not have the physical state marked on the label and was marked as <br /> containing loose pills and empty containers that once contained pharmaceuticals in the Warfarin family(IMG <br /> 4102-4103). <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> accurate information. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> FA0023487 PR0541025 SCO01 02/06/2019 <br /> EHD 22-03 Rev.09/22/16 Page 11 of 12 RCRA Large Quantity Hazardous Waste Generator OIR <br />
