The following is an itemized list of underground storage tank violations that have not been addressed for MARCH LANE 76 * as of February 18 , 2022 . Open violations from December 16 , 2021 inspection Violation #208 - Failure to comply with the designated operator inspection requirements . OBSERVATION : The designated operator failed to check and note all required items on the visual inspection reports . The incorrect and missing information included the following from the inspection reports : -DO report dated 3/2/2021 did not mark the mechanical float mechanisms used in UDCs box REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report . CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #209 - Failure to maintain a copy of the " Facility Employee Training Certificate" on -site . OBSERVATION : The Facility Employee Training Certificates dated 11 / 10/2021 , 3/31 /2021 , 3/2/2021 , 2/3/2020 and 1 /9/2020 all contain different initial training dates . REGULATION GUIDANCE : All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 , The Facility Employee Training Certificate shall contain accurate information for all employees trained . CORRECTIVE ACTION : Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #317 - Failure of the line leak detector to detect a 3 . 0 gph leak at 10 psi and restrict or shut off flow. OBSERVATION : The 87 & 91 line leak detector failed to detect a leak when tested . REGULATION GUIDANCE : All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi . CORRECTIVE ACTION : Immediately have a properly licensed , trained , and certified contractor repair or replace the failed leak detector ( LG 113-x listed , if applicable) and obtain a permit within one business day from the EHD , if needed . If the failed leak detector can 't be replaced immediately, there is a possibility that the 87 & 91 UST system may be red tagged to prevent fuel inputs . Page 2 of 3
