5. PUBLIC HEALTH SERVICES (Staff Contact: Mike Huggins, 468-3437) <br /> a. The underground farm tank must be removed and disposed with the appropriate manifest. All <br /> contamination found during removal must be reported to the San Joaquin County Environmental <br /> Health Division, Unit III. <br /> b. A pesticide testing report for Phase F is acceptable at this time. The school site may require <br /> additional testing after final grade and compaction. <br /> c. Prior to Final Map recordation, the transformer identified on Page 5 of the Condor Report(dated <br /> September 18, 1998) shall either be tested for PCB content or replaced, with appropriate <br /> documentation submitted by PG&E. <br /> d. For areas located within 500 feet of any pipeline, a preliminary site assessment shall be prepared <br /> prior to submittal of a tentative map by a qualified professional in compliance with the <br /> requirements of the San Joaquin County Public Health Services. If contamination is identified, an <br /> investigation and remediation shall be undertaken in accordance with the requirements of the <br /> County and the Regional Water Quality Control Board. <br /> 6. BYRON-BETHANY IRRIGATION DISTRICT(Staff Contact: Rick Gilmore, 510-634-3534) <br /> a. The developer must submit a plan to BBID and the MHCSD that indicates which agricultural <br /> areas are to be converted to urban development, along with an approximate time frame in which <br /> they will be converted. If the existing irrigation systems are affected by proposed construction, a <br /> revised irrigation service plan must submitted for approval by BBID. Upon approval, an <br /> Encroachment Permit will be issued by BBID for any construction activity within BBID facilities. <br /> b. Any proposed construction that interferes with BBID surface and/or subsurface drainage facilities <br /> shall require a Relocation and Improvement Plan, which will require the review and approval of <br /> BBID and in conjunction with the MHCSD. Upon approval, BBID will issue an Encroachment <br /> Permit to the developer. <br /> c. A Drainage Agreement between the MHCSD and BBID shall be entered into which will address <br /> existing agricultural surface"tailwater" drainage and subsurface"shallow groundwater" drainage. <br /> The Agreement will include specific provisions for the conveyance of drainage waters via the <br /> proposed MHCSD storm drain system. <br /> •y• <br /> Conditions of Approval -5 - SU-98-4 <br />
