RECEIVED ON 09/02/2022 VIA EMAIL <br /> 209 CCR 2715(c)(3),2715(c)(4) Failure to maintain a copy of the 'Facility Employee Training Certificate"on-site. <br /> OBSERVATION: The Facility Employee Training Certificate dated 9/1 71202 1 was missing CERS ID, city. zip code, <br /> phone number and Date of Assuming Responsibility as a Facility Employee. <br /> REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a repeat violation, Class II. UE0 <br /> Item#209: Provide an updated and complete training certificate <br /> Response: Facility Employee Training Certificate is attached. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not instal led/maintained,located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION. Schrader valves were open. but pointed up and not positioned in a way that leaks will be detected <br /> at the earliest opportunity in the 87 turbine sump <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity <br /> CORRECTIVE ACTION- Immediately contact a properly licensed. trained, and certified contractor to ensure that any <br /> releases In the piping will be detected at the earliest opportunity. <br /> Thos is a rer-eal 'violation Class II. <br /> #302: How was this corrected? <br /> Response: Corrected at the time of repairs. Monitoring Certification is attached. <br /> Hazardous Materials Business Plan <br /> 1 1 HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to appropriate personnel <br /> OBSERVATION:No training records for the Hazardous Materials Business Plan were available for review during the <br /> inspection. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material,including,but not limited to,familiarity with the following <br /> plans and procedures <br /> (A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons,property,or the environment. <br /> (C)Evacuation plans and procedures,including immediate notice,for the business site <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION.Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation <br /> 3 <br />
