OBSERVATION: Designated operator reports were not signed within the required time frame. -The 6/21/2021, 10/17/2021, 12/12/2021 and 3/7/2022 designated operator reports were not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. **************************************************************** OBSERVATION: The designated operator failed to check and note the correct test dates for the Spill Container testing, Overfill Prevention and Secondary Containment testing on the designated operator inspection reports dated 6/21/2021 to 3/7/2022. The correct dates are: -Spill Container Testing-5/26/2021 -Overfill Prevention-5/25/2021 (87, diesel, 91-1), 11/24/2021 (91-2) -Secondary Containment Testing-6/7/2021 REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 9/17/2021 was missing CERS ID, city,zip code, phone number and Date of Assuming Responsibility as a Facility Employee. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II.
