OBSERVATION: Schrader valves were open, but pointed up and not positioned in a way that leaks will be detected at the earliest opportunity in the 87 turbine sump. REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to ensure that any releases in the piping will be detected at the earliest opportunity. This is a repeat violation, Class II. 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. OBSERVATION: The 3000 gallon 91 VMI line leak detector failed to detect a leak when tested. REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. This is a repeat violation, Class II. 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. OBSERVATION: Overfill prevention testing was due in October 2021 and has not been conducted. REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is conducted within required time frames. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by May 6, 2022.
