The following is an itemized list of underground storage tank violations that have not been addressed for DIAMOND GAS AND FOOD MART as of August 24, 2022. Open violations from June 14, 2022 inspection Violation#101 -Failure to obtain a permit to operate from the EHD including but not limited to unpaid permit fees. OBSERVATION: Fees for the annual permit to operate the UST system were not paid until April 7, 2022. REGULATION GUIDANCE: No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. CORRECTIVE ACTION: Immediately contact the EHD accounting office to make arrangements for payment to obtain a permit to operate a UST system. CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a copy on site. 2)OBSERVATION:The most recent letter from the chief financial officer document in CERS dated 10/21/2021 net worth amount is incorrect. REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually.
