Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COOGC037H Site Location: 18129 E SOLA RD <br />Activity Summary <br />Activity Date Recorded by <br />06/25/2021 Received information from Carol Presto (EHD) concerning a chemical explosion at MANZO <br />this facility (the EHD notification came at approximately 3:50 pm). Called Chief <br />Bailey, with the Farmington Fire Department 209-639-5321. Chief Bailey reported <br />that a chemical explosion occurred at 16500-18129 Sola Road, Stockton, CA <br />when two chemicals were inadvertently mixed during a chemical delivery <br />operation. The two chemicals involved in the incident were reported to be sodium <br />chloride and etidronic acid solution. During the explosion, one 30-year old male <br />sustained injuries and chemical burns to the lower part of his body (-9% of the <br />body) and was taken to the San Joaquin General Hospital. Chief Bailey also <br />reported that during the incident, approximately 100-gallons of liquid mixture was <br />released onto the soil and flowed in the direction of the nearby irrigation ditch. <br />Chief Bailey reported that the impacted ground appeared to be smoking and was <br />hot to the touch (the heat was felt through the shoes when walking on it) in the <br />area of the release. No fire was reported and no evacuations are being required at <br />this time. During the incident, Simplot was delivering the chemicals and their <br />representatives were still on scene at this time. Simplot will be facilitating the spill <br />clean up, the contact information for clean up activities is Robert Blevins <br />(559-320-5305). Discussed the spill with John Pallen 208-867-2627, Simplot EHS <br />Manager. John reported that they are in the process of determining if the Cal <br />OSHA will need to be notified and if so, the scene may need to be preserved. John <br />indicated that their criteria for notification will be the hospitalization of the injured <br />individual. At this time Simplot is working on organizing clean up. Notified Melissa <br />Nissim (CUPA program coordinator), and M. Naidu (EHD assistant director). <br />Notified L. Called stand-by SJCPHO phone number, <br />481-4233, spoke with Mr. F. Zamora, provided incident information. Per Mr. <br />Zamora's request, ennailed the notification to fzamora1@sjgov.org.
