Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COOGC037H Site Location: 18129 E SOLA RD <br />Activity Summary <br />Activity Date <br /> <br />Recorded by <br />06/25/2021 Continuation from day time. BAKER <br />Met with Chief Bailey and fireman Matt with the Farmington fire department. <br />Fireman Matt showed me around the area. Sodium chlorite was mistakenly <br />added to an approximately 200 gallon black plastic tank of etidronic acid solution <br />during delivery causing an explosion of the tank. Approximately 200 gallons of <br />liquid was released on the north side of the tank and flowed into the drainage <br />ditch. Spill site on soil covered an approximate area of 15x20 feet. The drainage <br />ditch appeared to have water in it prior to the spill. Liquid in the ditch extended <br />approximately 50 feet. Soil immediately adjacent to where the tank was located <br />was steaming. Tony Ruff of Simplot on site upon arrival. PG&E de-energized line <br />from pole to pump. Tony Ruff said the plan for clean-up was to neutralize the area <br />with water and not doing any additional clean-up. Called Elena Manzo (Lead Sr. <br />REHS) to discuss clean-up. <br />Received call from John Pallen (Simplot) at 208-867-2627 and discussed clean-up <br />of site. He said they would cone/tape off area and he was in the process of getting <br />a clean-up company. Notifications were made to OSHA, OES and 911. <br />Generated complaint, emergency response record and prop 65. Wrote Notice to <br />Abate. Went over Notice to Abate with Tony Ruff. Emailed Notice to Abate to <br />john.pallen@simplot.com, Tony.Ruff@simplot.com and <br />robert.blevins@simplot.com. <br />Received second call from John Pallen. He said that Ancon would be on site on <br />6/26/21 at 8 am to begin clean-up. <br />Verified that area was coned and taped off before leaving site. <br />6:33 pm-Left site <br />7:00 pm-Left message for Fish & Wildlife notifying them of spill that occurred. <br />5pnn-7:06pm. <br />06/25/2021 Received complaint information from Elena Manzo (Lead Sr. RENS). Chemical <br />explosion of sodium chlorite and etidronic acid solution. Approximately 100 <br />gallons went into the irrigation ditch. Incident occurred at 18129 Sola Road in <br />Stockton. Robert Blevins (Simplot-559-320-5305) is on site. <br />Left for site at 4:20 pm. Arrived on site at 4:54 pm. Continued to after hours. <br />06/25/2021 Received a call from Fire Dispatch (a transferred phone call from the main EHD <br />line). Dispatch stated that there was a chemical explosion that occurred between <br />the addresses 16650 E Sola Rd and 18129 Sola Rd (cross street Mariposa, <br />heading toward Escalon). The explosion occurred after the mixing of 2 chemicals. <br />Chemicals from the explosion flowed into a nearby waterway. Fire chief on site <br />was Chief Bailey. <br />BAKER <br />PRESTO <br />06/25/2021 Received a number for Simplot contact on site from Chief Bailey. Called Simplot PRESTO <br />contact on site, Robert Blevins (559-320-5305), to verify if a spill report to Cal OES <br />was made for the chemical spill.
