Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: CO6QVK53B Site Location: 1813 MOFFAT BLVD, MANTECA, CA 95336 <br />Activity Summary <br />Activity Date <br />09/08/2021 On site and met with Dave Larson of PG&E and Ricardo Martinez of PSC. Per <br />Dave Larson, four transformers released <br />mineral oil (blue sticker) due to a vehicular accident. Per Dave, the transformers <br />are approximately 20-22 gallons each. Per <br />Dave, two transformers were almost empty and two were half full after the release. <br />Dave Larson stated that samples were <br />taken from each transformer. Per Dave and Ricardo, there were two impacted <br />areas from two poles. Per Dave and <br />Ricardo, one area was already cleaned before my arrival. I observed PSC <br />pressuring washing the second impacted area <br />(asphalt) and pumping the rinseate into their truck. Per Ricardo, absorbent was <br />applied onto the asphalt and swept. Then <br />the asphalt was cleaned with dawn and oil eater, scrubbed, and then pressure <br />washed. Per Ricardo, approximately 40-60 <br />gallons of rinseate was generated. Per Ricardo, the waste generated will be <br />labeled and secured on the PSC truck for the <br />night and will be transported to a PG&E facility at a later time. It was unclear at the <br />time of the inspection which PG&E <br />facility it would be transported to. Per Dave, it would be transported to the Manteca <br />location. Per Ricardo, it would be <br />transported to the Stockton location. I observed staining on the shoulder of the <br />road (on soil). Per Ricardo, PSC began <br />shoveling the area with plans to finish clean-up today. While shoveling, Ricardo <br />stated that they may need to excavate <br />deeper and will need to call USA Dig before they continue. The impacted soil was <br />covered with poly sheeting, weighed <br />down with soil, and secured with cones and caution tape. Per Ricardo, clean-up <br />will continue either Friday or Saturday. <br />Recorded by <br />VANG-LEE <br />Generated Notice to Abate and emailed it to R2SV@pge.com. <br />(Time: 7:48pm-10:06pm) <br />09/08/2021 Discussed complaint details with lead Elena Manzo and inspector Carol Presto. VANG-LEE <br />09/09/2021 Received a call from Alex Steele informing me that the spill cleanup was ALANIZ <br />completed. Notified lead inspector. <br />09/09/2021 Created Prop 65 PRESTO <br />09/09/2021 Drove to site to see backfill after receiving phone call from John Alaniz that they PRESTO <br />were ready to backfill. Arrived on site but PG&E and clean up companies were not <br />there. Comcast was on site fixing their fibers. Called Alex Steele who said Backfill <br />will be conducted another time. Alex also stated on the phone that the sample <br />results came back as non-detect. Walked around the and took photos of site as it <br />currently is. Staining can be seen on the railroad side of Moffat Blvd across from <br />1st Light Energy. Emailed Alex the photo of staining on the railroad side of Moffat <br />Blvd. and requested additional clean up be conducted on that side. <br />09/10/2021 Discussed complaint with Vicky Vang-Lee, RENS, and Melissa Nissim, CUPA PC.
