Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: CO6QVK53B Site Location: 1813 MOFFAT BLVD, MANTECA, CA 95336 <br />Activity Summary <br />Activity Date Recorded by <br />09/10/2021 Called and spoke to Alex Steele of PG&E notifying him that Carol Presto, REHS, FLORIDO <br />was unable to verify the completion of the clean-up, specifically the impacted soil <br />that was excavated, since there was no one on-site when she visited on 9/9/21 <br />and the excavation was covered with plastic. Also informed Alex that based on <br />Carol's photos, the spill appeared to have crossed Moffit Blvd and impacted the <br />soil road shoulder, on the side of Moffitt with the railroad tracks and across the <br />street from 1st Light Energy facility. Alex stated he will have PSC contact me when <br />they will be on-site today to finish the clean-up, so that I may go out to the site to <br />verify. Received a call from Juan Delgado of PSC stating he was on his way to the <br />site. Requested Juan to call me once he was on-site and knew what time he would <br />be done with the cleanup so I could go out to verify the cleanup was complete. <br />Received a call from Alex Steele stating he was on-site with PSC and that I could <br />head out to the site now to verify the cleanup. Left EHD. Arrived on-site with <br />Melissa Nissim, CUPA PC, and met with Alex Steel and Juan Delgado. PSC was <br />still excavating the two spill locations on both shoulders of Moffit Blvd. Observed <br />PSC complete the clean-up. Verified the clean-up was complete. Two 55-gallon <br />drums of waste were generated today. Took photos. Left site. Arrived back at <br />EHD. <br />09/10/2021 Updated program coordinator and day time on-call inspector regarding complaint VANG-LEE <br />status. <br />09/13/2021 Completed photo log and updated inspector Carol Presto. VANG-LEE <br />09/16/2021 Created photo log. Printed out 5104. Printed Emergency Response record. Placed PRESTO <br />map with spill location with Emergency Response. Printed Notice to Abate. Put <br />together in a packet. <br />09/20/2021 Emailed Alex Steele, R2SV@pge.com, to provide a copy of the sample results to PRESTO <br />EHD. <br />09/30/2021 Called Alex Steele, 209-337-8902, to follow up on sample results. Alex requested PRESTO <br />an email sent to him so he can respond with the results tomorrow morning when <br />he's in the office. Sent Alex an email at R2SV@pge.com to follow up on sample <br />results. <br />10/01/2021 Received an email from Alex Steele (R25v@pge.com) with a copy of the sample <br />results. The sample results all shows non detect. Printed email and sample results <br />for file. <br />Pending update to ER Response with time of arrivals and departure, then can <br />abate complaint. <br />10/04/2021 The ER times of arrival and departure were in the narrative entry. Added to ER <br />Record. Reviewed log2, Elianna Florido took photos on site of completed clean up <br />on 9/10/2021. Pending photos from Elianna then can close out complaint packet. <br />11/09/2021 Generated photo log of clean up on 9/10/2021. Emailed photo log to Carol Presto, <br />REHS. <br />11/09/2021 Reviewed and revised my 9/10/21 narrative. <br />11/09/2021 Received photo log from inspector visit on 9/10/2021.
