HAZARDOUS MATERIALS INCIDENT REPORT <br />CHP407E <br />SEQUENCE OF EVENTS: On April 30, 2021, at approximately 1440 hours, a Ford <br />F650 tow truck was parked along the right shoulder of 1-5 northbound, south of Eight <br />Mile Road. The tow operator was standing on the back of tow truck adjusting his load. <br />During this time, a Hino two axle bobtail glass truck was traveling northbound on I-5 <br />approaching the Ford F650's location, while traveling in the #2 lane, at approximately 45 <br />mph. The driver of the Hino bobtail failed to keep his attention focused on the road, <br />allowing the Hino bobtail to drift off of the roadway and onto the right shoulder. The <br />Hino bobtail collided with the back of the tow truck, ejecting the tow operator off of the <br />back of the truck and onto the grassy right shoulder. The tow operator suffered minor <br />injuries as a result. After the initial impact, both the tow truck and the Hino bobtail <br />continued in a north-easterly direction across the right shoulder and down the <br />embankment. The impact ruptured the saddle fuel tank of the tow truck, causing <br />approximately 45 gallons of diesel fuel to leak out onto the dirt and grass embankment. <br />Unit 62-505, Officer C. Sandford, ID 22356, was dispatched to the scene, as was <br />American Medical Response (AMR) Medic 35 for patient transport. Unit 62-505 arrived <br />on scene at 1457 hours, assuming Incident Command (IC) responsibilities. Stockton Fire <br />responded, arriving at 1502 hours and Woodbridge Fire also responded, arriving on scene <br />at 1456 hours. At 1530 hours upon evaluating the scene, Unit 62-505 discovered the <br />ruptured fuel tank and subsequent diesel fuel spill onto the dirt shoulder. Unit 62-505 <br />advised Stockton Communications Center (STCC) of the hazardous materials spill. Unit <br />62-S3, Sergeant B. Butler, ID 13238, copied the radio traffic and requested a Cal.-Trans. <br />supervisor response to the scene. At 1615 hours, the #3 lane was closed for vehicle <br />removal. Call-Trans. handled the traffic control for the scene. At 1627 hours, Unit 62-S3 <br />arrived on scene and took over IC responsibilities. A SIG ALERT was issued to advise <br />motorists of the lane closure and subsequent traffic delay. An OES number (21-2304) <br />was requested from Environmental Health, as well as a response from the San Joaquin <br />County Office of Emergency Services (OES). At 1643 hours, Unit 62-S3 also made <br />notification to Unit 62-L1, Lieutenant B. Waughan, ID 17299, regarding the hazardous <br />materials spill. <br />College Oak towing responded for the recovery of the Ford F650, at the owner's request <br />and Sam Berri Heavy Haul towing responded out for the recovery of the Hino bobtail, at <br />the owner's request. At 1715 hours, J. Armandaraz, a Hazardous Material Specialist <br />from Cal.-Trans. responded to the scene. He was made the Site Safety Officer by Unit <br />62-S3. J. Armandarez spoke with the company manager for the Ford F650, D. Clones, <br />and provided him with a list of contractual environmental clean up companies that could <br />handle the spill removal and excavation of the contaminated soil. D. Clones was able to <br />contract out with US Ecology for the environmental clean up. The area to be excavated <br />was marked by Cal.-Trans. and after the two involved vehicles had been removed by their <br />perspective tow companies, the scene was determined safe by J. Armandarez at 1830 <br />hours. At 1903 hours, the roadway was clear.
