Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00054015 Site Location: 9999 S AUSTIN RD <br />Activity Summary <br />Activity Date <br />07/08/2021 <br />Recorded by <br />Received complaint information from the front counter regarding 20 barrels of FLORIDO <br />nitric acid causing a white cloud at Forward Landfill, 9999 S Austin Rd in Manteca, <br />reported by Lathrop-Manteca Fire Dept. (Fire). <br />Called Fire dispatch and was referred to Battalion Chief Mike !sham. Per Mike, he <br />just arrived on-site, is going to make contact with the landfill facility representative, <br />assess the incident location, and call me back. <br />Called and left a voice message for Jeremy Welch, Forward Landfill environmental <br />contact listed on CERS. Received a call back from Mike who states the following: <br />he is not requesting a joint team call-out; the drums are located in the north-west <br />area of the landfill, and there is a haze above the barrels; the barrels were <br />disposed at the landfill by Metal Finishing Solutions, located at 1325 El Pinal Dr. in <br />Stockton; 100 feet in all directions from the barrels and 1/4 mile downwind from <br />the barrels is evacuated; and Fire will be suiting up in level A PPE to get close to <br />the barrels to determine the exact number of containers and types of chemicals. <br />Called and discussed complaint with Ruben Ramirez (209-969-9952), Landfill <br />Operations Manager, who states he is currently acting as the landfill's <br />environmental contact, since the previous environmental contact recently left the <br />company. Informed Ruben that the landfill would be the responsible party for this <br />complaint and will have to ensure the clean-up is completed, since the complaint <br />occurred on the landfill's property. Ruben states Metal Finishing Solutions will be <br />hiring a clean up company and is the responsible party. <br />Received a call from Mike who states there are twenty-one 55-gallon plastic drums <br />and sixty-two 5-gallon containers labeled as hazardous materials; the containers <br />were overturned; two of the 55-gallon drums released an unknown amount of what <br />is believed to be sodium hydroxide (UN3266) and a corrosive liquid containing <br />nitric acid (UN1760), based on the drum labeling, onto a 12 inch by 12 inch area of <br />soil; there is also a drum labeled as UN2031 (Nitric Acid); the liquid on one drum <br />lid has a pH of 14; drums and containers have been uprighted, except for one <br />drum that is buried beneath garbage; most of the drums and containers appear to <br />be empty, however several drums and containers may still contain hazardous <br />material inside; Fire deconed into a 10ft by 10ft area of soil; the barrels are no <br />longer off-gassing; no exposures occurred; and the evacuation order will not be <br />lifted until the area is cleaned. Mike stated he will stay on-site until EHD arrives. <br />Informed Natalia Subbotnikova, Solid Waste Unit Ld Sr, regarding the complaint. <br />Emailed Dr. Maggie Park, Public Health Officer, a notification regarding the <br />complaint and status. <br />Arrived on-site with Melissa Nissim, CUPA Program Coordinator, and Elena <br />Manzo, Ld Sr REHS, and met with Mike and Ruben. Took photos of the incident <br />and decon areas from afar. Received additional photos of the barrels from Fire. <br />Informed by Fire and Ruben that Metal Finishing Solutions has hired All Clean <br />Hazardous Waste Removal Inc. to conduct the clean-up. At 11:18am, called and <br />spoke to Terry McGuinness (707-536-6360) of All Clean, who states he is working <br />on getting a crew to the site today. Informed Terry to let me know when the <br />clean-up crew will be on-site so that I can be on-site, be informed about the <br />clean-up plan, and verify the clean-up is complete. Terry agreed. Fire left the site. <br />Ruben stated he will remain on-site to ensure that no one gets near or disturbs the <br />incident area. Informed Ruben to let me know when the clean-up crew arrives <br />on-site. Ruben agreed. Left site. <br />Arrived at EHD. At 12:19pm, called Terry because I noticed I missed his 12 pm <br />call. Terry stated he will arrive on-site at 1:45pm today by himself to assess the <br />5105.rpt <br /> Page 2 of 9
