Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COGKTPW7A Site Location: 2201 W WASHINGTON ST <br />Activity Summary <br />Activity Date <br />01/04/2021 <br />Recorded by <br />Received an email notification from Mike Caliguire (F&VV) that he will go to the MURO <br />spill area on Monday, Jan. 4, 2021 because the removal of the barge has been <br />completed. I called Mike Caliguire who said he was ten minutes away from the <br />site, but will not be staying long as he was only going to take a look at the site. I let <br />him know I will be verifying as well. <br />Spoke to Steven Bender (Port of Stockton) and he said he wasn't aware that they <br />were done and that Mike would be going out there. I asked him if he can send me <br />a pin, since it is a difficult site to find. Steven sent me a pin. <br />Headed out to Building 20 at the port. There was no one on site. I looked at the <br />water, no oily sheen or odor were observed. No debris or boom was inside or <br />outside the water. <br />Called John Cowan (Ross Island Sand and Gravel) to let him know I was there <br />and that the cleanup was adequate. Let him know we will need the waste amount <br />and a copy of the disposal record. John told me that it will be added up in the <br />presence of the Fish and Wildlife OSPR and will provide as soon as he has it. <br />Called Mike Caliguire to ask if he had any concerns after visiting the spill site. He <br />said he didn't have any. <br />01/04/2021 Generated photo log. MURO <br />01/25/2021 Ross Island Sand and Gravel - Emailed OSPR Mike Calguire. MURO <br />01/25/2021 Updated Elena Manzo on complaint's current status. MURO <br />01/26/2021 Received response from Mike Caliguire (F & W) letting me know he has not MURO <br />received the waste information from the responsible party, but will notify me once <br />he does. <br />01/29/2021 Received a copy of an invoice from accounting that returned. MURO <br />Updated responsible party information on the complaint form 5104 and took to <br />accounting. <br />02/16/2021 Received disposal records via email from John Cowan. MURO <br />Uniform Hazardous Waste Manifest 021990765 JJK for 725 gallons Non-RCRA <br />Hazardous Waste Liquid (Oily water) shipped on 1-12-2021. <br />03/01/2021 Received disposal manifest from John Cowan (Ross Island Sand and Gravel) via MURO <br />email. <br />Uniform Hazardous Waste Manifest#: 014709873 FLE for the disposal of one CM <br />20-yard Non-RCRA Hazardous Waste Solid (Absorbent booms c/w oil) ship date <br />01-27). <br />Printed and added to file. <br />04/05/2021 Retrieved from my inbox an invoice with a yellow mailing label indicating it was <br />returned to sender and a 5104 complaint form for reference. Reviewed narrative <br />to confirm the mailing address was updated, per request from John Cowan (Ross <br />Island Sand and Gravel). <br />Confirmed the address to be correct as given by John Cowan on 12-03-2020. <br />MURO <br />5105 rpt Page 4 of 5
