Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COGKTPW7A Site Location: 2201 W WASHINGTON ST <br />Activity Summary <br />Activity Date <br />12/03/2020 Called John Cowan, 360-790-3165, for status update. He reported the following: <br />- all fuel was removed from the rig and currently sotered on site for Fish and <br />Wildlife to assess <br />- Responsible Party mailing address changed to 6605 SE Lake Rd., Portland OR <br />97222 <br />I asked him to notify me when they are finished with removal of the barge so that I <br />can verify the area of release and waste generated. He told me he will notify me <br />since Mike Calguire will also be out to verify once removal of the barge is <br />complete. <br />12/03/2020 Discussed the return of the Prop 65 with Elena Manzo. <br />I received the new mailing address from John Cowan so I attached the new <br />information and request for mailing to the new address. <br />Placed in 'maililngs' box. <br />12/14/2020 Called Steven Bender (Port of Stockton's Industrial Hygenist) 209-323-9008. He <br />said there is no update to report. Ross Island Sand and Gravel decided to salvage <br />the barge. The petroleum was removed. The barge is being cut up piece by piece. <br />His guess is it may take 5- 6 weeks for them to be done with removing the barge. <br />Recorded by <br />MURO <br />MURO <br />MURO <br />12/16/2020 Mapped location on Envision Connect GIS Map. MURO <br />12/18/2020 Emailed Mike Calguire (Fish & Wildlife) requesting notification of his final site visit MURO <br />to the Port of Stockton's diesel spill area per phone call on 11-04-2020, he stated <br />he will also visit when all barge fragments, boom, and absorbent pads are <br />removed for waste quantification. Lead Senior Elena Manzo (EHD) requested that <br />I visit the diesel spill area once cleanup has been completed. <br />MURO 12/21/2020 Received reply from Mike Calguire. Mike stated that he will be on site upon <br />completion of the job to quantify the hazmat lost to State Waters and all <br />absorbents used during the response. <br />12/23/2020 Emailed Steven Bender, John Cowan, and Mike Calguire. Requested contact <br />information for the contractor onsite doing the cleanup and removal of barge. I <br />also asked that they notify EHD once completion is anticipated well in advance so <br />that I or another inspector can go to the site to verify cleanup. <br />12/30/2020 Reviewed field notes and photos. <br />12/30/2020 Completed photo log. <br />12/31/2020 Searched the internet for phone number for Ballard Diving and Salvage.
