Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CORLW5SRC Site Location: 2201 W WASHINGTON ST <br />Activity Summary <br />Activity Date <br />07/21/2020 Called John Cowan, Steve Crawford, and Steven Bender for cleanup information. <br />07/21/2020 Updated Elena Manzo on the complaint and plan for cleanup today. <br />Gave compliant information, photos, contact information to John Alaniz who is on <br />call today and will be verifying cleanup. <br />Discussed complaint with Elena Manzo and Garrett Backus and the facility's <br />routine inspection which is due for them. Garrett Backus determined he could <br />verify the cleanup tomorrow when he goes out to the facility's routine inspection. <br />07/22/2020 Verified completion of spill cleanup. The dredge barge has been refloated. <br />Absorbent pads and boom have been pulled out of the water and bagged. No <br />sheen was observed on the water around the barge. Two bags of oily absorbent <br />pads and boom waste were generated by the cleanup. I asked John Cowan to <br />send the disposal record to us for the absorbent waste. He's having Ramos <br />Environmental pick up the waste. <br />Recorded by <br />MURO <br />MURO <br />BACKUS <br />07/22/2020 Completed Prop 65, Emergency Response Record, and site map and photo log. MURO <br />07/27/2020 Received call from John Cowan 503-936-0944. He told me he has copies of <br />disposal records from Ramos Environmental that he would like to drop off. He will <br />be coming in today, 7-27-2020 around noon. <br />John Cowan stopped by. I walked out to meet him so he can had-deliver the <br />paperwork for the disposal of the hazardous. <br />MURO <br />Reviewed paperwork. Placed with complaint packet. <br />08/04/2020 Emailed John Cowan (Ross Island Sand and Gravel) at: jcowan@ridredge.com to MURO <br />ask what was done with the fuel that was in the vessel. <br />08/13/2020 Received a call from John Cowan. He was responding to my email with the MURO <br />question of the fuel on board the vessel. They pumped the fuel out and added to <br />the liquid waste generated from cleanup. The boat will continue to operate as no <br />issues were identified. <br />09/23/2021 Reviewed complaint in preparation for abatement procedure. Gathered documents MURO <br />and generated list of items still required for abatement. <br />10/11/2021 Complaint file preparation for abatement. MURO <br />01/11/2022 Generated photo log of initial site visit. Printed an updated narrative log, 5104, and MURO <br />5105 reports. Compiled complaint file and scanned to a pdf. Submitted complaint <br />for review for abatement. <br />01/24/2022 Located final hazardous waste manifest copy for 021989043JJK, printed included MANZO <br />in the abated complaint package. <br />5105.rpt <br /> Page 2 of 2
