WESTERN PUMP <br /> Petroleum & Lubrication Equipment Specialists <br /> Fall Restraint <br /> Fall restraint means that workers will be kept clear of the fall hazards , also referred to as "dog <br /> on a leash". When utilizing fall restraint, the following requirements shall be satisfied : <br /> 1 . Anchor points must be rated at 3 , 000 pounds . <br /> 2. No more than one worker shall be attached to a single anchor point. <br /> 3. Self Retracting Lanyards (SRL's) shall not be used for fall restraint. <br /> 4. Energy absorbers shall not be required . <br /> 5 . Lanyards shall be of fixed length and as short as possible . <br /> 6 . The entire system must be inspected and documented by a Competent Person prior to <br /> use . <br /> Guardrail Systems Requirements <br /> 1 . Top-rail must be at a height of 42-45 (California) . <br /> 2. Mid-rails must be at a height of 21 inches . <br /> 3. Top-rails must be able to withstand 200 pounds of outward or downward force . <br /> 4. Mid-rails must withstand a force of 150 pounds . <br /> 5 . No steel or plastic banding shall be used for a top-rail . <br /> 6. Wire rope top-rail must be flagged every 6 feet. <br /> 7. All wire rope used for perimeter protection must be at least 3/8 inch nominal diameter. <br /> 8. Manila , plastic, or synthetic rope rails must be inspected frequently. <br /> 9. The entire system must be inspected and documented by a Competent Person prior to <br /> use . <br /> Cover Requirements <br /> 1 . Covers located in roadways and vehicular aisles shall be capable of supporting , <br /> without failure , at least twice the maximum axle load of the largest vehicle expected to <br /> cross over the cover. <br /> 2. All other covers shall be capable of supporting , without failure , at least twice the weight <br /> of workers , equipment, and materials that may be imposed . <br /> 3. All covers shall be secured when installed to prevent accidental displacement by wind , <br /> equipment, or workers . A cover is not secured if it can be picked up with moderate <br /> effort. <br /> 4 . All covers shall be marked in paint with the words "CAUTION — OPENING - DO NOT <br /> REMOVE" . <br /> 5.
